The collision resulted in multiple injuries and one fatality. The AOC's injury investigation team conducted an investigation to develop measures for prevention of similar incidents in the future. Data collection involved a comprehensive review of medical records, a detailed survey of the accident scene, assessment of vehicle conditions, interviews with survivors and first responders, and analysis of injury factors according to Haddon’s matrix model.</p> <p> The investigation revealed that there was one death at scene; a 47-year-old motorcycle sidecar driver, was reported death at the scene. Additionally, a 52-year-old taxi driver was injured. The incidence was attributed to the reckless driving behavior of both the taxi driver and the driver of the private car. The severity of the collision was likely to increase by high-speed driving on the descent of the overpass in combination with poor visibility and the lack of warning signs or speed-reducing measures, such as speed bumps on the bridge.</p> Phongsak Nitikaroon Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 108 113 Investigation on Sedan Car Accident Crashed into the Back of an 18-Wheel Semi-trailer Truck Carrying a Container on Bang Kruai-Sai Noi Road, Mueang Nonthaburi District, Nonthaburi Province https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3807 <p> The Ambulance Operation Center (AOC) of Phra Nang Klao hospital, Nonthaburi province, received a report of an accident in May 2024. The accident involved a sedan rear-ending an 18-wheel semi-trailer truck carrying a container. Two fatalities were reported at the scene. The accident occurred in front of a Noodle Restaurant on Bang Kruai-Sai Noi Road in Mueang Nonthaburi district, Nonthaburi province, during a period of post-rain conditions. The injury investigation team of the Ambulance Operation Center (AOC), Phra Nang Klao hospital, conducted an investigation into the injuries and fatalities caused by this road accident to identify preventive measures to avoid future accidents. Data collection was gathered by using medical records, site surveys of the accident scene, vehicle condition assessments, interviews with the survivors, and first responders. Analysis of contributing factors to injuries was performed by using the Haddon’s Matrix Model.</p> <p> The investigation revealed that two fatalities were reported at the scene. The deceased included the driver of the sedan, aged 57, and a passenger, aged 95. The accident is presumed to have been caused by the sedan driver’s reckless behavior, including high-speed driving on wet and slippery road conditions after rainfall, combined with rapid lane changes. Despite the vehicle's braking assistance system, the wet road surface likely caused the car to skid uncontrollably, resulting in a severe impact that damaged the vehicle extensively, including the passenger cabin. The severity of the injuries led to fatalities for both the driver and the passenger.</p> Sunti Angkanasopit Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 114 119 The Effectiveness of Implementing COVID-19 Nursing Practice Guidelines in the Emerging Infectious Disease Critical Care Unit at Sakon Nakhon Hospital https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3653 <p> This quasi-experimental research aimed to evaluate the effectiveness of COVID-19 critical care nursing practice guidelines in the Emerging Infectious Disease Critical Care Unit at Sakon Nakhon Hospital and to assess nurses' satisfaction with these guidelines. The sample consisted of 13 registered nurses and 40 COVID-19 critical patients, divided into pre- and post-implementation groups of 20 patients each. The research instruments consisted of COVID-19 critical care nursing practice guidelines, nursing activity checklist, and satisfaction assessment form. All three instruments were validated with a Content Validity Index (CVI) of 0.80. Reliability was analyzed using the Cronbach's alpha coefficient; the nursing activity checklist had a reliability of 0.85, and the satisfaction assessment form had a reliability of 0.87. Data were analyzed using descriptive statistics and Chi-square test.</p> <p> The results showed that 1) after implementing the guidelines, the incidence of complications in patients decreased significantly (p &lt; .05), including severe sepsis (from 20% to 5%), acute kidney injury (from 20% to 10%), mortality rate (from 10% to 5%) and acute respiratory failure (from 10% to 5%) and 2) the registered nurses had the highest level of satisfaction with the guidelines (Mean = 4.38, S.D = 0.51). These findings demonstrate that the guidelines were effective in reducing complications and well-accepted by practitioners.</p> Jeerawan Sirimontri Kanitta Hunghuan Tasanee Daekhunthod Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 1 12 The Prophylactic Effects of Dexamethasone on Postoperative Sore Throat and Hoarseness after Double Lumen Endobronchial Tube Intubation: a Randomized Controlled Trial https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3779 <p> Postoperative sore throat (POST) and hoarseness are common complications following double-lumen endobronchial tube (DLT) intubation. This randomized, double-blinded, controlled trial aimed to evaluate the efficacy and safety of prophylactic dexamethasone in 94 patients undergoing thoracic surgery requiring DLT intubation at Sakon Nakhon Hospital between November 2022 and May 2023. Patients were randomly allocated to receive either dexamethasone 0.2 mg/kg or normal saline intravenously after anesthesia induction. Primary outcomes were the incidence and severity of POST and hoarseness at 1 and 24 hours postoperatively. Secondary outcomes included adverse effects.</p> <p> The dexamethasone group showed significantly lower incidence of POST at both 1 hour (2.17% vs 28.26%, p &lt; 0.001) and 24 hours (15.22% vs 56.52%, p &lt; 0.001) postoperatively. The relative risk reduction at 24 hours was 73.07%, with a number needed to treat of 2.42. Hoarseness was also significantly reduced at 24 hours (6.52% vs 30.43%, p = 0.003). No significant adverse effects were observed during the 2-week follow-up period. Given these findings and its favorable risk-benefit profile, prophylactic intravenous dexamethasone 0.2 mg/kg is recommended for routine use in eligible patients undergoing thoracic surgery with DLT intubation.</p> paphatsara phuangpraphan Ariyachart kalawa Phatcharawe wongsapan Phongsri nuanmanee Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 13 22 Effects of using Save me leaflet media and Save me application on knowledge and behavior of early adolescents to prevent unwanted pregnancy https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3275 <p>This quasi-experimental research aimed to compare the knowledge and behavior scores in preventing unwanted pregnancy in early adolescents between the group that received knowledge promotion using Save me leaflet media and the group that received knowledge promotion using Save me application. The sample group was 1-3 secondary school students at Phattha Pittayakhom School, Tha Uthen District, Nakhon Phanom Province, 50 people per group, totaling 100 people. The research instruments were Save me leaflet media and Save me application developed by the researcher. The data collection instruments were the knowledge and behavior assessment form in preventing unwanted pregnancy. Knowledge scores and behavior scores were compared between the groups using the Independent t-test.</p> <p>The results showed that after the experiment, 1) the average knowledge scores in the group that used Save me leaflet media (Mean = 13.45, S.D. = 1.12) and the experimental group that used Save me application (Mean = 13.90, S.D. = 1.35) increased to a high level. From the hypothesis testing with t-test, it was found that the average knowledge score of both groups was not different. 2) The average behavior score in the group using the Save me leaflet media (Mean = 19.36, S.D. = 3.96) increased at a moderate level, while the group using the Save me application (Mean = 26.62, S.D. = 5.02) increased at a high level and was statistically significant at p&lt;0.05.</p> <p>Therefore, using the Save me leaflet media and using the Save me application can promote knowledge in early adolescents to increase at the same level. The sample consisted of 147 third-year nursing students in the academic year 2023 at Boromarajonani College of Nursing Udon Thani and 10 supervising instructors. The research instrument was a 5-point rating scale questionnaire assessing generosity behaviors in critical care nursing, with an Item-Objective Congruence Index (IOC) of 0.67-1.00. The Cronbach's alpha reliability coefficients for student self-assessment and supervisor evaluation forms were 0.96 and 0.97, respectively. Data were analyzed using descriptive statistics including mean and standard deviation, and differences were compared using an independent t-test.</p> <p> The study found that both nursing students' self-assessment and supervising instructors' evaluation of generosity behaviors in caring for critically ill patients were at high levels across all aspects, with morality and ethics receiving the highest mean scores. Overall, there was no significant difference between students' self-assessment and supervising instructors' evaluation (t = 1.80, p = 0.07). However, when comparing individual aspects, students rated themselves significantly higher than their supervising instructors in three areas: ability, confidence building, and commitment (p &lt; 0.01). Regular breast self-examination (BSE) is crucial for early detection of breast abnormalities. This study aimed to 1) study the knowledge of breast cancer in older women 2) explore BSE behavior of older women, and 3) determine the relationship between knowledge of breast cancer and BSE behavior among older women. This research was a cross-sectional correlational study conducted between October 2020 and January 2021. A simple random sampling was used to select 152 older women. Data were collected using the demographic questionnaire, the knowledge of breast cancer questionnaire, and the BSE behavior questionnaire. The reliability the knowledge of breast cancer and the BSE behavior questionnaire were 0.79 and 0.82, respectively. Data were analyzed using the descriptive statistics and Spearman's rank correlation coefficient.</p> <p> The results showed that the level of knowledge of breast cancer was poor (<img id="output" src="https://latex.codecogs.com/svg.image?\bar{x}" alt="equation" /> = 11.80, S.D. = 4.07), and the level of BSE behavior was low (<img id="output" src="https://latex.codecogs.com/svg.image?\bar{x}" alt="equation" /> = 10.32, S.D. = 9.20). Moreover, the relationship between the knowledge of breast cancer and the BSE behavior among older women was statistically significant (r<sub>s</sub> = .389, p &lt; .001). The samples were selected using a multi-stage random sampling, consisting of: 1) 30 caregivers who completed at least 70 hours of elderly care training and had at least 1 year of experience in caring for dependent elderly, 2) 30 dependent elderly with Activities of Daily Living (ADL) scores between 0-11, and 3) 30 family members of dependent elderly. Data were collected using the personal information questionnaires, knowledge assessment forms, perception of caregiver role questionnaires, role satisfaction of caregiver questionnaires, elderly ADL assessment forms, and family satisfaction questionnaires. The overall reliability coefficient of instrument was 0.87. Data were analyzed using descriptive statistics and t-test.</p> <p> The research findings revealed that the development model for enhancing capabilities of caregivers for dependent elderly comprised: 1) enhancement of knowledge in elderly care, which included health and illness assessment, assessment of problems and needs of dependent elderly and infection prevention and control in the new normal era, 2) development of elderly care skills, including daily care for dependent elderly, specific care activities such as: pressure ulcer wound care, nasogastric tube feeding and physical rehabilitation, 3) development of communication skills and teamwork and 4) empowerment and coping strategy development for caregivers. After model implementation, the caregivers showed significantly improved knowledge, role perception, and role satisfaction, and the family members had significantly higher satisfaction with caregivers' performance (p &lt; 0.05). However, there was no significant difference in the elderly's ADL scores before and after model implementation.</p> <p> Research recommendations suggest continuous development of caregivers' capabilities to enhance their potential and confidence in performing their roles. The sample consisted of 74 Care Givers in Kham Ta Kla District, selected through simple random sampling. Data was collected using questionnaires from September to October 2024. The data was analyzed using descriptive statistics including frequency, mean, percentage, standard deviation, and inferential statistics including Pearson's correlation coefficient and stepwise multiple regression analysis to predict factors affecting the effectiveness of elderly care performance among caregivers for dependent elderly.</p> <p> The results showed that caregivers had good overall knowledge scores for dependent elderly care ( = 14.20, S.D. = 2.48) and good overall skill scores for dependent elderly care ( = 59.91, S.D. = 6.81). The social support factors were at a good level overall ( = 106.62, S.D. = 6.65). The knowledge (r = 0.246, p = 0.036), community acceptance (r = 0.430, p &lt; 0.001), support from sub-district health promoting hospitals (r = 0.478, p &lt; 0.001), support from local administrative organizations (r = 0.252, p = 0.032), and support from network hospitals (r = 0.424, p &lt; 0.001) were significantly correlated with the overall effectiveness of dependent elderly care performance of Care Givers at the 0.05 level. Furthermore, support from sub-district health promoting hospitals, community acceptance, support from network hospitals, and knowledge collectively predicted 61.7% of the variance in Care Givers' effectiveness in providing care for dependent elderly (R<sup>2</sup> = 0.617, p &lt; 0.05). This study demonstrates that Care Givers who receive support from sub-district health promoting hospitals, community acceptance, support from network hospitals, and possess knowledge show improved effectiveness in providing care for dependent elderly.</p> Wangai Promkawong Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 70 84 The Relationship between Organization Climate and Nursing Leadership with Resilience Quotient of Professional Nurse under Emerging Disease Situation at Middle-Level Hospital in Regional Health 2 https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3548 <p> The purpose of this descriptive research was to examine the relationship between organizational climate, nursing leadership, and resilience quotient among professional nurses under emerging disease situations in middle-level hospitals in the Regional Health 2. The study sample comprised 166 professional nurses, 69 samples from Wichian Buri Hospital and 97 samples from Sri Sangwan Sukhothai Hospital. The samples were selected through stratified sampling from April to June 2024. The research instrument was a questionnaire for personal characteristics of professional nurses, organizational climate perceptions, nursing leadership perspectives and resilience quotient of professional nurse under emerging disease situation validated by five experts and yielded a content validity index (CVI) of .93. The reliability of the questionnaire for organizational climate, nursing leadership, and resilience quotient were assessed using the Cronbach's alpha coefficient, resulted in values of .92, .95, and .94, respectively. Data analysis employed the descriptive statistics and Spearman's rank correlation coefficient. The statistical level of .05 was considered significant.</p> <p> The findings revealed a moderate positive correlation between organizational climate and resilience quotient (r<sub>s</sub> = .508), as well as between nursing leadership and resilience quotient (r<sub>s</sub> = .485) among professional nurses under emerging disease situations in middle-level hospitals in the Regional Health 2. Both correlations were statistically significant at the p &lt; .001 level. The study's key recommendations are strategically designed to optimize the work environment for nursing personnel, enabling them to execute their professional responsibilities with maximum effectiveness and sustained commitment, particularly in current and under emerging disease situation.</p> runiya trakunboon Wanchanok Juntachum Copyright (c) 2024 Sakon Nakhon Hospital 2024-12-27 2024-12-27 27 3 85 95 A Survey Research: Happiness Levels and Coping Behaviors with Work-Related Problems among Personnel at Sisaket Rajabhat University https://he05.tci-thaijo.org/index.php/JSakonNakHosp/article/view/3451 <p> This survey research aimed to study the happiness levels, coping behaviors with work-related problems, and the relationships between demographic characteristics, happiness, and coping behaviors among personnel at Sisaket Rajabhat University from July 1<sup>st</sup> to September 30<sup>th</sup>, 2023. The sample consisted of 138 personnel selected through simple random sampling. The research instruments included the HAPPINOMETER assessment and a work-related coping behavior questionnaire. Data were analyzed using percentage, mean, Chi-square, and Fisher's exact test.</p> <p> The research results revealed that the level of happiness at work was high at 56.5%. Dimensions 1-7, which include good health, relaxation, kindness, spirituality, family, society, and pursuit of knowledge, were all at high levels. Dimensions 8-9, covering financial health and work satisfaction, were at moderate levels. The overall average happiness level was high ( <img id="output" src="https://latex.codecogs.com/svg.image?\bar{x}" alt="equation" /> = 3.47, S.D. = 0.51). Regarding problem-focused coping behaviors at work, the top two practices at high levels were learning from past experiences for self-development and resolving situations based on experience. The overall implementation was at a moderate level (<img id="output" src="https://latex.codecogs.com/svg.image?\bar{x}" alt="equation" /> = 1.88, S.D. = 0.57). For emotion-focused coping behaviors, the top two practices at high levels were increasing activities such as listening to music, playing sports, gardening, and getting more sleep.
