Retrospective study on the treatment outcomes of frozen shoulder by hydrodilatation with steroid via rotator interval under ultrasound guidance

Abstract

Background


Inflammation and fibrotic change are components of pathogenesis process of frozen shoulder. Hydrodilatation with steroid via rotator interval (RI) under ultrasound guidance is a treatment for frozen shoulder that affect both of these components. Studies have reported that RI is the major pathologic site of frozen shoulder and may be the primary structure to receive treatment. The author intends to investigate this treatment further for its treatment potential and for the fact that this treatment is not currently widespread in Thailand.


Objective


To report the results of treating frozen shoulder by hydrodilatation with steroid via RI under ultrasound guidance and identify factors influencing the results of this intervention.


Methods


A retrospective observational study with a pre-post design


Results


The number of frozen shoulder patients treated with hydrodilatation with steroid via (RI) under ultrasound guidance included in the analysis of this retrospective study is 30 patients. Results showed that pain is reduced with statistical significance (P<0.001) both at night and during normal daily activities at 2 weeks and 6 weeks follow up and all shoulder range of motions (ROM) also increased with statistical significance (P<0.001) at 2 weeks and 6 weeks follow up. Factors influencing the increase in shoulder ROM at 6 weeks follow up were identified. Patients with diabetes mellitus (DM) have more difficulty in improvement of forward flexion with statistical significance (Coefficients (95% CI) = -0.427 (-0.840, -0.014); P-value = 0.043).


Patients with duration of symptoms over 3 months have better improvement of external rotation with statistical significance (Coefficients (95% CI) = 0.420 (0.171, 0.669); P-value = 0.002).


Conclusion


Hydrodilatation with steroid via RI under ultrasound guidance resulted in pain reduction and quick restoration of shoulder ROM, thus it is considered as an interesting treatment option for frozen shoulder. Additionally, factors influencing the shoulder ROM are DM and duration of symptoms.
