มุมมองผู้มีส่วนได้ส่วนเสียต่อคุณค่าทางเศรษฐศาสตร์และความท้าทายของระบบดูแลระยะยาวผู้สูงอายุในประเทศไทย
Stakeholders’ Perspectives on Economic Value and Care Challenges in Long-Term Care for Older Adults in Thailand

ภูมิหลังและเหตุผล: ประเทศไทยเข้าสู่สังคมสูงวัยอย่างรวดเร็ว ทำให้ความต้องการการดูแลระยะยาว ท่ามกลางข้อจำกัดเชิงโครงสร้างและทรัพยากร จึงจำเป็นต้องใช้มุมมองผู้มีส่วนได้ส่วนเสียในการกำหนดนโยบายการดูแลระยะยาวที่ยั่งยืน
วัตถุประสงค์: เพื่อศึกษามุมมองของผู้มีส่วนได้ส่วนเสียเกี่ยวกับคุณค่าทางเศรษฐศาสตร์ ความต้องการ และอุปสรรคของการดูแลระยะยาวสำหรับผู้สูงอายุ
วิธีการศึกษา: ใช้วิธีการวิจัยเชิงคุณภาพโดยเทคนิคการสนทนากลุ่ม ผู้เข้าร่วมสนทนากลุ่มได้รับการคัดเลือกแบบเฉพาะเจาะจงจากผู้มีส่วนได้ส่วนเสีย 4 กลุ่ม ผู้เข้าร่วมการศึกษา จำนวน 48 คน เก็บรวบรวมข้อมูลโดยการสนทนากลุ่ม บันทึกเทป และจดบันทึกการสนทนา และนำข้อความที่ได้จากการถอดเทปแบบคำต่อคำ ประโยคต่อประโยควิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหาตามความเป็นจริง และการวิเคราะห์แก่นสาร
ผลการศึกษา: พบประเด็นแก่นสาระหลัก 3 ประเด็น ได้แก่ (1) การดูแลระยะยาวเป็นทั้งภาระทางเศรษฐกิจของครัวเรือนและการลงทุนเชิงระบบ (2) ความต้องการการดูแลที่ยังไม่ได้รับการตอบสนอง โดยเฉพาะการสูงวัยในถิ่นที่อยู่เดิม การดูแลผู้มีภาวะสมองเสื่อม และศักยภาพกำลังคน และ (3) อุปสรรคเชิงระบบด้านการเงิน ความเหนื่อยล้าของผู้ดูแล การประสานงานข้ามภาคส่วน และบริบททางวัฒนธรรม
สรุปผลและข้อเสนอแนะ: การดูแลระยะยาวสำหรับผู้สูงอายุยังมีช่องว่างด้านการคุ้มครองทางการเงิน การดูแลผู้มีภาวะสมองเสื่อม ศักยภาพกำลังคน และการบูรณาการบริการ จึงควรเร่งพัฒนาระบบการดูแลระยะยาวแบบบูรณาการระดับชาติที่ยั่งยืน เน้นบริการชุมชน การสูงวัยในถิ่นที่อยู่เดิม และความร่วมมือข้ามภาคส่วน
