Abstract

This descriptive cross-sectional study aimed to investigate the factors influencing the decision-making process for utilizing denture services among older adults at Papayom Hospital, Phatthalung Province. The sample consisted of 81 older adults selected through systematic random sampling. The research instrument was a 68n item questionnaire developed by the researcher based on a literature review and Andersen's Behavioral Model of Health Services Utilization. The questionnaire was divided into four parts: personal data, barriers to service access, psychological factors, and the decision to receive denture services. The content validity of the questionnaire was assessed verified by three experts, yielding an Item–Objective Congruence (IOC) index ranging from .70 to 1.00. Reliability was tested using Cronbach’s alpha coefficient with the values the sections on barriers to service access, psychological factors, and the decision to receive denture services being at an acceptable level (α = .79 - .88). Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation, and inferential statistics, specifically hierarchical multiple regression analysis. The results revealed that:

1. The overall level of decision-making to utilize denture services among the elderly was high (M = 2.67, S.D. = 0.39).

2. Factors that significantly influenced the decision to utilize denture services included perceived need for dentures (OR = 2.27, p-value = 0.008), high expectations regarding service utilization (OR = 2.11, p-value = 0.015), positive attitudes toward denture treatment (OR = 1.98, p-value = 0.021), and moderate and high levels of motivation for obtaining dentures (OR = 1.57, p-value = 0.041 and OR = 2.34, p-value = 0.009, respectively), as well as barriers to denture service utilization (p-value < .05).

The findings suggest that promoting awareness of the necessity of dentures, fostering positive attitudes toward denture treatment, and reducing barriers to accessing services such as shortening waiting times and providing transportation support may help increase opportunities for elderly individuals in the community to access denture services.
