A Retrospective Study on the Association of Illicit Drugs in Urine with Injury Patterns and Severity Among Road Traffic Fatalities at the Forensic Medicine Center, Ratchaburi Hospital

Background: Drug-involved traffic accidents represent a growing public health crisis, particularly in Thailand, where methamphetamine abuse is highly prevalent. However, epidemiological evidence linking urine drug detection, organ-system injury patterns, and overall severity using the Injury Severity Score (ISS) among traffic fatalities remains limited.
Objective: To investigate the prevalence of illicit drugs in urine, evaluate organ-specific injury patterns, and analyze the relationship between drug detection and ISS among road traffic fatalities.
Methods: A retrospective cross-sectional study was conducted on 200 autopsied road traffic fatality cases at the Forensic Medicine Center, Ratchaburi Hospital (2022–2025). Urine drug screening, AIS-85 injury patterns, and ISS were recorded. Statistical analyses included Fisher's Exact test, Chi-square test, Independent t-test, and Kruskal-Wallis test (p < 0.05).
Results: Urine drug positivity was 22.0% (n = 44). Methamphetamine was most prevalent (63.6% of positive cases), with polysubstance use observed in 27.3% of drug-positive fatalities. Organ-system injury incidence showed no statistically significant differences between the drug-positive and drug-negative groups (p > 0.05 for all systems). Mean ISS did not differ significantly between drug-positive (38.72 ± 21.11) and drug-negative (41.06 ± 22.63) groups (p = 0.541). Furthermore, mutually exclusive subgroup analysis (methamphetamine alone, cannabis alone, other single drugs, and polysubstance use) revealed no significant differences in ISS (p = 0.892).
Conclusion: Illicit drugs, particularly methamphetamine, are highly prevalent among road traffic fatalities. However, urine drug detection was not significantly associated with specific internal organ injury patterns or overall injury severity (ISS) in fatal traffic collisions.
