ผลของรูปแบบการพยาบาลโดยส่งเสริมการมีส่วนร่วมของครอบครัว ต่อผลลัพธ์ทางคลินิก ของผู้ป่วยเด็กโรคปอดอักเสบ ในหอผู้ป่วยกุมารเวชกรรม โรงพยาบาลสันป่าตอง จังหวัดเชียงใหม่

บทนำ: โรคปอดอักเสบในเด็กเป็นปัญหาสำคัญที่ส่งผลต่อการเจ็บป่วยและการกลับมารักษาซ้ำ โดยเฉพาะเมื่อผู้ดูแลขาดความรู้และทักษะที่เหมาะสม การส่งเสริมให้ครอบครัวมีส่วนร่วมจึงเป็นแนวทาง
ในการพัฒนาคุณภาพการดูแล การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลของรูปแบบการพยาบาลที่ส่งเสริมการมีส่วนร่วมของครอบครัวต่อความรู้ ความสามารถ และผลลัพธ์ทางคลินิกของผู้ป่วยเด็กโรคปอดอักเสบ
วิธีการศึกษา: การวิจัยกึ่งทดลองแบบสองกลุ่มวัดก่อน–หลัง กลุ่มตัวอย่างคือผู้ป่วยเด็กอายุ 1 เดือนถึง 5 ปี และผู้ดูแลหลัก จำนวน 60 คู่ แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 30 คู่ กลุ่มทดลองได้รับรูปแบบการพยาบาลที่พัฒนาขึ้น เครื่องมือประกอบด้วยแบบทดสอบความรู้ แบบประเมินความสามารถ แบบประเมินการมีส่วนร่วม และแบบบันทึกข้อมูลทางคลินิก วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และสถิติเชิงอนุมาน ได้แก่ paired samples t-test, independent samples t-test, Chi-square test และ Fisher’s exact test
ผลการศึกษา: หลังการทดลอง ทั้งสองกลุ่มมีคะแนนความรู้และความสามารถเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p < .001) เมื่อเปรียบเทียบภายในกลุ่ม และเมื่อพิจารณาคะแนนการเปลี่ยนแปลง พบว่ากลุ่มทดลองมีการเพิ่มขึ้นมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญในทุกตัวแปร ได้แก่ ความรู้ (95% CI: −2.67 ถึง −1.19; p < .001) ความสามารถ (95% CI: −2.58 ถึง −0.49; p = .005) และการมีส่วนร่วมของครอบครัว (95% CI: −3.91 ถึง −1.69; p < .001) โดยมีขนาดอิทธิพลระดับสูง ขณะที่ผลลัพธ์ทางคลินิก ได้แก่ ระยะเวลานอนโรงพยาบาล การได้รับ HFNC การย้ายเข้า ICU และการกลับมารักษาซ้ำภายใน 28 วัน ไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p > .05)
สรุปผล: รูปแบบการพยาบาลที่ส่งเสริมการมีส่วนร่วมของครอบครัวช่วยเพิ่มความรู้ ความสามารถ และการมีส่วนร่วมของผู้ดูแลหลักอย่างมีนัยสำคัญทางสถิติ แม้ยังไม่พบความแตกต่างของผลลัพธ์ทางคลินิก แต่แนวโน้มที่ดีขึ้นอาจสะท้อนถึงศักยภาพของรูปแบบการดูแลนี้
