บทคัดย่อ

บทนำ ผู้ป่วยโรคเรื้อรังกลุ่มชาติพันธุ์ในพื้นที่ห่างไกล เช่น อำเภอแม่แจ่ม จังหวัดเชียงใหม่ เผชิญอุปสรรคหลายด้านต่อการใช้ยาอย่างต่อเนื่อง ทั้งปัจจัยทางสังคมวัฒนธรรม ภาษา และการเข้าถึงบริการสุขภาพ ซึ่งส่งผลโดยตรงต่อผลลัพธ์ทางคลินิกและภาระโรค

วิธีการศึกษาการวิจัยเชิงพรรณนาแบบภาคตัดขวาง เก็บข้อมูลจากผู้ป่วยโรคเรื้อรังกลุ่มชาติพันธุ์ จำนวน 180 รายระหว่างเดือนพฤษภาคม–ตุลาคม 2568 ด้วยการสุ่มแบบหลายขั้นตอน ประเมินความร่วมมือในการใช้ยาด้วยเครื่องมือ TMARS และ TMARS-VAS วิเคราะห์ด้วย Chi-square/Fisher's Exact Test และ Multiple Logistic Regression

ผลการศึกษากลุ่มตัวอย่างส่วนใหญ่เป็นชาติพันธุ์กะเหรี่ยง (ร้อยละ 56.11) ประกอบอาชีพเกษตรกรรม มีรายได้ต่ำกว่า 10,000 บาท/เดือน พฤติกรรมเชิงลบที่พบบ่อย ได้แก่ ลืมกินยา (ร้อยละ 38.3) และกินยาไม่ตรงเวลา (ร้อยละ 32.8) ปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญ ได้แก่ ชาติพันธุ์ ศาสนา อาชีพ สถานภาพสมรส ชนิดโรค จำนวนยา ระยะเวลาที่ป่วย ระยะทาง วิธีเดินทาง ความเชื่อเรื่องยาสมุนไพร และความคล่องแคล่วทางภาษาไทย

สรุปผลระยะทางจากบ้านถึงโรงพยาบาลเป็นปัจจัยพยากรณ์ความไม่ร่วมมือในการใช้ยาที่มีนัยสำคัญเพียงปัจจัยเดียว โดยผู้ป่วยที่อยู่ห่างเกิน 10 กิโลเมตร มีความเสี่ยงสูงกว่ากลุ่มที่อยู่ใกล้ถึง 4.62 เท่า (Adjusted OR = 4.62, 95% CI = 1.85–11.57 p = 0.001) ชี้ให้เห็นว่าอุปสรรคเชิงโครงสร้างมีผลเหนือปัจจัยส่วนบุคคล
