Abstract

Introduction: This study aimed to analyze the relationship between blood pressure (BP) control and other risk factors with stroke incidence among patients with hypertension at Doi Lo Hospital, Chiang Mai.

Methods: A retrospective analytical study was conducted using medical records of 3,897 patients. Data were analyzed using descriptive statistics and multiple logistic regression to identify independent predictors of stroke, with the significance level set at 0.05.

Results: The mean age of the participants was 64.1±12.9 years. The incidence of stroke was 117 cases, accounting for 3.0% during the study period. Multivariable analysis showed that patients with uncontrolled BP had a 1.47-fold increased trend in stroke risk compared to the controlled group, although not statistically significant at the 0.05 level (Adjusted OR = 1.47, 95% CI: 0.99-2.17, p = 0.054). Other risk factors that significantly predicted the occurrence of stroke included age (Adjusted OR = 1.03, 95% CI: 1.02-1.05, p < 0.001), diabetes mellitus (Adjusted OR = 1.66, 95% CI: 1.05-2.60, p = 0.028), and females having a lower risk than males (Adjusted OR = 0.53, 95% CI: 0.37-0.78, p = 0.001).

Conclusion: Blood pressure control level showed no statistically significant association with stroke occurrence at the 0.05 level, although a trend toward increased risk was observed in the uncontrolled group. Advanced age, diabetes mellitus, and male sex were significant independent predictors. Clinical practitioners should therefore emphasize proactive monitoring and close follow-up for elderly and diabetic patients to reduce future stroke incidence.
