Assessment of Adherence to the Practice Guideline of High Alert Drug in Debaratana Vejjanukula Hospital Commemorating Her Royal Highness Princess Maha Chakri Sirinthorn's 60 th Birthday Anniversary

Background
Debaratana Vejjanukula Hospital Commemorating Her Royal Highness Princess Maha Chakri Sirinthorn’s 60th Birthday Anniversary has developed a high-alert drug management system to investigate the incidence of medication errors, adverse drug events, and the effectiveness of established high-alert medication protocols.
Methods
This study is a retrospective descriptive study, conducted in three distinct phases: identifying operational problems and obstacles, system development and evaluating the developed system.
Results
The study results showed that the medication with the highest incidence of medication errors was Warfarin (66.67%), followed by Nicardipine injection (33.33%). Furthermore, 11 adverse events were identified: constipation from Morphine injection (27.27%), bruising from Warfarin (27.27%), and phlebitis from Norepinephrine injection (18.18%). The medication error rate decreased to 0.05 per thousand prescriptions. Regarding the results of adherence to high-alert drug practice guidelines, the prescribing, pre-dispensing and dispensing processes achieved a compliance rate of over 80%. However, the administration process reached only 67.44%.
Conclusion
In conclusion, the development and evaluation of High-alert drug management systems foster safety and effectively reduce the risk of medication errors.
