Role of Emergency Bypass in Moyamoya Disease : Case Report
Abstract
ผู้ป่วยหญิงไทยอายุ 58 ปี ญาตินำส่งโรงพยาบาลด้วยอาการอ่อนแรงซีกขวา 7 ชั่วโมงก่อนมาโรงพยาบาล ตรวจร่างกายพบว่ามีอ่อนแรงซีกขวา ใบหน้าซีกขวาอ่อนแรง ได้รับการทำ brain computed tomography (CT) ได้รับการวินิจฉัยเป็น acute ischemic stroke และ CT perfusion พบว่า มี mismatch ระหว่าง penumbra area และ core infarct ของสมองซีกซ้าย ผู้ป่วยได้รับการทำ cerebral angiography พบว่ามี stenosis ของ right supraclinoid internal carotid artery (ICA), right M1 segment ของ middle cerebral artery (MCA), left A1 segment ของ anterior cerebral artery (A1) และ basal collateral vessel ร่วมกับมีการตีบตันของ left supraclionoid ICA หลังทำพบว่า อาการอ่อนแรงซีกขวาของผู้ป่วยดีขึ้นจาก motor power grade 2 มาเป็น grade 3+ หลายชั่วโมงต่อ มา อาการอ่อนแรงแย่ลงเหลือ grade 0 และมี global aphasia ซึ่งสัมพันธ์กับช่วงที่ความดันซิสโตลิกน้อยกว่า 130 mmHg เอ็กซเรย์คอมพิวเตอร์สมองซ้ำ ไม่พบเลือดออก ผู้ป่วยได้รับการวินิจฉัยเป็น Moyamoya disease อาการอ่อนแรงสัมพันธ์กับภาวะ hypoperfusion ซึ่งเป็นข้อบ่งชี้ในการผ่าตัดต่อหลอดเลือดสมอง superficial temporal artery - middle cerebral artery (STA-MCA) bypass เป็นกรณี เร่งด่วน
