Development of a clinical assessment model to prevent aspiration in psychiatric patients The context of a tertiary hospital in the Northeastern region
Abstract
This study, conducted as a Routine to Research (R2R) initiative, aimed to investigate the situation and problems of aspiration pneumonia and analyze contextual issues.
The research also focused on developing a clinical assessment model to prevent aspiration in psychiatric patients within a tertiary hospital setting in the Northeastern region of Thailand. A purposive sample of 138 participants was selected between June 2023 and June 2024, divided into two groups: Group 1 consisted of 124 psychiatric patients at risk of aspiration, with varying levels of consciousness and communication abilities, and Group 2 included
14 professional nurses working in the patient wards. The research tools used included: 1) personal data questionnaires, 2) GCS assessment forms, 3) RASS assessment forms, 4) CIWA-Ar assessment forms, 5) patient readiness assessment forms before initiating swallowing, and
6) nurse satisfaction questionnaires. Data analysis involved quantitative methods, including percentage, mean, and standard deviation, as well as qualitative analysis through in-depth interviews using content analysis. The research findings revealed that: 1. A clinical assessment model was developed to prevent aspiration in psychiatric patients, comprising daily patient physical assessments, dysphagia tests, and water swallow tests. 2. The incidence of aspiration pneumonia in patients was reduced. 3. Nurse satisfaction with the model was high (Mean = 3.41, S.D. = 0.60)
