Development of an Empowerment Program for Caregivers of Patients with Continuous Ambulatory Peritoneal Dialysis (CAPD) in Mahasarakham Community

Abstract

การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อ (1) ศึกษาลักษณะทั่วไป ภาระการดูแล และความต้องการของผู้ดูแลผู้ป่วยที่ได้รับการล้างไตทางช่องท้องชนิดถาวร (Continuous Ambulatory Peritoneal Dialysis: CAPD) ในจังหวัดมหาสารคาม (2) พัฒนาโปรแกรมเสริมพลังอำนาจผู้ดูแลที่เหมาะสมกับบริบทชุมชน และ (3) ประเมินประสิทธิผลของโปรแกรมต่อความรู้ ทักษะการดูแล ความมั่นใจในตนเอง ภาระการดูแล สุขภาพจิต และความพึงพอใจของผู้ดูแล โดยดำเนินการเป็น 2 ระยะ ได้แก่ ระยะที่ 1 การวิเคราะห์สถานการณ์และพัฒนาโปรแกรม (R1–D1) และระยะที่ 2 การนำโปรแกรมไปใช้และประเมินผล (R2–D2)

กลุ่มตัวอย่างคือผู้ดูแลหลักของผู้ป่วย CAPD จำนวน 80 คน คัดเลือกแบบเฉพาะเจาะจง โดยโปรแกรมดำเนินการต่อเนื่องเป็นเวลา 6 สัปดาห์ ประกอบด้วยกิจกรรมสำคัญ ได้แก่ การให้ความรู้ การฝึกทักษะปฏิบัติจริง การเสริมสร้างความมั่นใจ การแลกเปลี่ยนเรียนรู้ระหว่างผู้ดูแล และการสนับสนุนต่อเนื่อง  เก็บข้อมูลก่อนและหลังการทดลองใช้ด้วยแบบสอบถามมาตรฐาน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Paired t-test และการวิเคราะห์เนื้อหา

ผลการศึกษาพบว่า ผู้ดูแลส่วนใหญ่เป็นเพศหญิง อายุเฉลี่ย 47.79 ปี และใช้เวลาดูแลผู้ป่วยมากกว่า 8 ชั่วโมงต่อวัน ภายหลังเข้าร่วมโปรแกรม คะแนนเฉลี่ยด้านความรู้ ทักษะการดูแล และความมั่นใจในตนเองสูงขึ้นอย่างมีนัยสำคัญทางสถิติ (p < .001) ในขณะที่คะแนนเฉลี่ยด้านความเครียด ความรู้สึกโดดเดี่ยว ภาระทางการเงิน และเวลาพักผ่อนส่วนตัวที่ลดลง ลดลงอย่างมีนัยสำคัญทางสถิติ (p < .05) ผู้ดูแลมีความพึงพอใจต่อโปรแกรมโดยรวมอยู่ในระดับมากที่สุด โดยเฉพาะด้านการสื่อสารของผู้สอนและการนำไปใช้จริง

สรุปผลการศึกษา โปรแกรมเสริมพลังอำนาจผู้ดูแลผู้ป่วย CAPD ที่พัฒนาจากความต้องการจริงของชุมชนและบูรณาการแนวคิดการเสริมพลังอำนาจร่วมกับความเชื่อมั่นในตนเอง สามารถเพิ่มศักยภาพของผู้ดูแล ลดภาระการดูแล และส่งเสริมสุขภาวะทางจิตใจได้อย่างมีประสิทธิผล ควรนำโปรแกรมไปประยุกต์ใช้ในระบบบริการสุขภาพระดับปฐมภูมิ และขยายผลสู่พื้นที่อื่นที่มีบริบทใกล้เคียง
