Development of Care Model for Type 2 Diabetes Patients Kaeng Leang Chan Health Promotion Hospital, Mueang District, Mahasarakham Province

ABSTRACT
Diabetes leads to organ dysfunction and failure, which can result in death. This research and development aimed to study the problem, develop a model, and evaluate the model. The sample was purposively selected, consisting of 30 diabetic patients, and 5 people participated in the model development. The research tools consist of two parts: research instruments such as focus group discussions and models of diabetic patient care. Data collection tools included a diabetes knowledge questionnaire and a self-care behavior questionnaire. Data were analyzed using descriptive statistics and inferential statistics.
The study found that problems included difficulty controlling blood sugar levels and a lack of knowledge in assessing health and monitoring symptoms after complications occurred. The care model includes educating people about the disease, treatment, and promoting self-care behaviors such as exercise and dietary control. The results showed that knowledge and self-care behaviors improved significantly (p-value <0.05).
Recommendations: Research findings indicate that the developed model has the potential to educate and promote self-care behaviors. It should be implemented in community-level healthcare facilities and applied to patients with chronic diseases and other risk factors to confirm the model's coverage.
