Effects of D-METHOD discharge planning program on knowledge, behavior and Readmission in chronic obstructive pulmonary disease patientsat Phanomsarakham Hospital, Chachoengsao Province

The research was a quasi-experimental one-group design, measured before and after the experiment. The objectives were to compare the mean knowledge scores and the mean behavior scores in managing exacerbations and to compare the mean readmissions within 28 days after using the program. There were 27 patients with chronic obstructive pulmonary disease who were admitted to the male inpatient ward of Phanomsarakham Hospital, Chachoengsao Province between April and September 2024. The research instruments consisted of a personal data questionnaire, a knowledge and behavior assessment form in managing exacerbations and readmissions, and the D-METHOD discharge planning program of the Nursing Division, Ministry of Public Health. Data were analyzed using descriptive statistics and compared using a paired t-test.

The results of the study found that the sample group after receiving the D-METHOD discharge planning program had higher mean knowledge scores and mean behavior scores in managing exacerbations than before receiving the program. The mean readmissions within 28 days after discharge (readmissions) decreased before receiving the program. Statistically significant at the 0.05 level ( t = 5.68 p = 0.00, t = 4.96 p = 0.01 and t = -11.76 p = 0.01 ), respectively. Conclusion: This research used the discharge planning guidelines for patients with chronic obstructive pulmonary disease according to the D-METHOD model at Phanomsarakham Hospital to help patients with chronic obstructive pulmonary disease have better knowledge and behavior and to reduce readmissions to the hospital.
