การประเมินผลการตรวจวิเคราะห์การติดเชื้อไวรัสตับอักเสบซี (HCV) ในโรงพยาบาลพุทธโสธร โดยวิธีการตรวจ anti-HCV, HCV Duo กับ HCV RNA

บทคัดย่อ
ที่มาและความสำคัญ: การติดเชื้อไวรัสตับอักเสบซีเป็นปัญหาสาธารณสุขที่สำคัญ การวินิจฉัยที่รวดเร็วและราคาถูกจึงมีความจำเป็น การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อประเมินความสอดคล้องของผลการตรวจวินิจฉัยการติดเชื้อไวรัสตับอักเสบซีด้วย rapid test, HCV Duo กับ HCV RNA Viral Load และเพื่อใช้การตรวจ HCV core antigen ทดแทน HCV RNA Viral Load ในการวินิจฉัยผู้ติดเชื้อรายใหม่ในโรงพยาบาลพุทธโสธร
วิธีการดำเนินงานวิจัย: ตัวอย่างที่ใช้ศึกษาเป็นซีรัมของผู้ป่วยโรงพยาบาลพุทธโสธร ที่ได้ตรวจคัดกรอง anti-HCV ด้วย BiolineTM HCV มีผลการตรวจเป็นบวก จำนวน 195 ราย และตรวจ HCV Duo ซึ่งเป็นการตรวจ anti- HCV และ HCV core antigen จากเครื่อง Cobas e801 ใช้น้ำยา Elecsys® HCV Duo เปรียบเทียบกับการ ตรวจ HCV RNA จากเครื่อง Cepheid GeneXpert ใช้น้ำยา Xpert® HCV RNA Viral Load
ผลการศึกษาวิจัย: จากการเปรียบเทียบผลการตรวจด้วยวิธี HCV core antigen กับ HCV RNA Viral Load พบว่ามีความไว 73.74% ความจำเพาะ 91.67% ค่าทำนายผลบวก 90.12% และค่าทำนายผลลบ 77.19% ผลการวิเคราะห์ความถูกต้องและแม่นยำจาก Area Under the Receiver Operating Characteristics (AUROC) ระหว่างการตรวจทั้งสองวิธีดังกล่าวเท่ากับ 0.83
สรุปผลการศึกษาวิจัย: การตรวจการติดเชื้อด้วย HCV RNA Viral Load กับ HCV core antigen มีความแม่นยำและถูกต้องสอดคล้องกัน ซึ่งสามารถใช้เป็นวิธีในการยืนยันการติดเชื้อไวรัสตับอักเสบซีในผู้ป่วยรายใหม่ได้ หากปรับแนวทางการส่งตรวจ HCV RNA Viral Load เฉพาะรายที่ HCV Duo ให้ผลบวก ใน anti-HCV แต่ HCV core antigen เป็นลบ หรือคนไข้กลุ่มเสี่ยง จะสามารถลดค่าใช้จ่ายในการตรวจวินิจฉัยการติดเชื้อด้วย HCV RNA Viral Load ถึง 40% อีกทั้งยังช่วยให้คนไข้ได้รับการรักษาที่รวดเร็ว เพื่อบรรลุวัตถุประสงค์ของกรมควบคุมโรคโดยกำจัดไวรัสตับอักเสบซีให้หมดไปภายใน พ.ศ. 2573
