PATIENT AND FAMILY SATISFACTION WITH NURSES' ETHICAL BEHAVIORS IN PALLIATIVE CARE WARDS OF A TERTIARY HOSPITAL

ภูมิหลัง: การดูแลแบบประคับประคองมุ่งเน้นบรรเทาความทุกข์ทรมานและศักดิ์ศรีความเป็นมนุษย์ ซึ่งพยาบาลต้องเผชิญกับประเด็นจริยธรรมที่ซับซ้อน การประเมินความพึงพอใจของผู้รับบริการต่อพฤติกรรมจริยธรรมจึงเป็นตัวชี้วัดสำคัญในการพัฒนาคุณภาพมาตรฐานการพยาบาลระยะท้ายในสถานพยาบาลระดับตติยภูมิ
วัตถุประสงค์: เพื่อประเมินและเปรียบเทียบระดับความพึงพอใจของผู้ป่วยและครอบครัวต่อพฤติกรรมด้านจริยธรรมของพยาบาลในหอผู้ป่วยที่ให้การดูแลแบบประคับประคอง
วิธีการศึกษา: การวิจัยเชิงสำรวจแบบภาคตัดขวาง ในกลุ่มตัวอย่างผู้ป่วยและครอบครัวผู้ดูแลหลักจำนวน 207 ราย
จาก 18 หอผู้ป่วยในโรงพยาบาลตติยภูมิแห่งหนึ่ง คัดเลือกโดยการสุ่มอย่างเป็นระบบ เครื่องมือวิจัยคือแบบสอบถามความพึงพอใจต่อพฤติกรรมด้านจริยธรรมของพยาบาล จำนวน 20 ข้อ (ค่าความเชื่อมั่นสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.94) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และเปรียบเทียบความแตกต่างระหว่างกลุ่มด้วยสถิติ Independent Samples t-test
ผลการศึกษา: พบว่าความพึงพอใจต่อพฤติกรรมด้านจริยธรรมของพยาบาลโดยรวมอยู่ในระดับสูงมาก (x = 4.68, SD = 0.52) เมื่อพิจารณารายมิติพบว่า มิติภาพลักษณ์และความน่าเชื่อถือมีคะแนนเฉลี่ยสูงสุด (x = 4.75, SD = 0.49) ส่วนพฤติกรรมด้านการรับฟังและช่วยคลี่คลายปัญหา และการปลอบโยนให้กำลังใจมีคะแนนเฉลี่ยต่ำที่สุด (x = 4.60 และ 4.61 ตามลำดับ) ทั้งนี้ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติของระดับความพึงพอใจระหว่างกลุ่มผู้ป่วยและกลุ่มครอบครัวในทุกรายการพฤติกรรม (p > 0.05)
สรุป: พยาบาลในหอผู้ป่วยที่ให้การดูแลแบบประคับประคองมีพฤติกรรมด้านจริยธรรมที่ดีเยี่ยมตามการรับรู้ของผู้รับบริการ อย่างไรก็ตามควรมีการเสริมสร้างทักษะการสื่อสารเพื่อการบำบัด การรับฟังเชิงลึก และการสนับสนุนด้านจิตใจอย่างต่อเนื่อง เพื่อยกระดับการดูแลอย่างเป็นองค์รวมของการดูแลแบบประคับประคองให้มีประสิทธิภาพสูงสุด
