CONTINUING OF PALLIATIVE CARE MODEL WITH PARTICIPATION OF MULTIDISCIPLINARY TEAMS, COMMUNITY NURSES AND NETWORK IN YANGSISURAT DISTRICT, MAHASARAKHAM PROVINCE

Abstract

This action research aimed to develop a model and evaluation of the continuing of Palliative Care model in Yangsisurat district.296 samples were community nurses, community network, end of life patients and their families. The research conducted from October 2016 - March 2019. Research tools were interview questionnaire, evaluation form, observation forms. Qualitative data were analyzed by content analysis and quantitative data by mean and percent.

The study results showed that the continuing of palliative care model with participation of multidisciplinary teams were ; 1) Developing the potential of care and knowledge of community nurses and community network partners 2) Establishing CPG in hospitals to community, with roles in each profession 3) Develop nursing care process consist of 5 CNPG 4) Define expressway channels for drug and medical equipment supplies support 5) Organize a forum to learn and remove lessons lead to development periodically improve. The evaluation found that during from 2017 - 2019 the patients to receive ACP 92.66, 96.66, 98.20% respectively. According to reduce pain 76.67, 83.87, 80.00% respectively, regurgitation nausea & vomiting 68.75, 78.57, 83.33% respectively recession depression 78.57, 87.50, 90.00% respectively and increased good death 80.20, 95.28, 96.66% respectively.
