Development and Validation of an Instrument Measuring Knowledge and Health Beliefs Regarding Fluid Overload in Patients Undergoing Hemodialysis

Fluid overload is a major complication in hemodialysis patients, with an incidence of 8–49%. Self-management plays a crucial role in preventing and controlling fluid overload, and patients’ knowledge and health beliefs influence it. Having a reliable measurement tool is essential for effective nursing care planning. This research and development study aimed to develop and examine the quality of a tool for measuring knowledge and health beliefs regarding fluid overload in end-stage renal disease patients undergoing hemodialysis. The questionnaire was developed based on a literature review and the Health Belief Model framework. Five experts assessed the content validity. The tool was pilot-tested with 15 patients. Reliability was analyzed using Kuder-Richardson 20 (KR-20) for the knowledge questionnaire and Cronbach’s alpha coefficient for the health belief questionnaire. The developed tool consists of three parts: 1) demographic information, 2) a 20 item multiple-choice knowledge questionnaire on fluid overload (three options per item), and 3) a 20 item health belief questionnaire on fluid overload using a 5 point Likert scale. The content validity index (CVI) ranged from 0.94 to 0.98. The KR-20 reliability of the knowledge questionnaire was 0.79, and Cronbach’s alpha coefficient for the health belief questionnaire was 0.81. The developed tool demonstrates acceptable content validity and reliability and can be used to assess knowledge and health beliefs regarding fluid overload in hemodialysis patients.
