Development of Denture Services Delivery System at Sub-district Health Promoting Hospitals using the Changhan Model for the Elderly in Changhan District, Roi Et Province

Thailand is transitioning into an aging society, leading to an increased demand for prosthetic dental services. However, service accessibility remains limited due to prolonged waiting times, causing some older adults to seek denture services from non-dentist providers, which results in oral health complications. This action research aimed to develop a denture service delivery model for older adults in sub-district health promoting hospitals, Changhan District, Roi Et Province. The sample consisted of 75 older adults aged 60 years and above with clinical indications for complete or partial dentures, selected through purposive sampling. Data were collected from October 2023 to September 2025, assessing service accessibility and patient satisfaction regarding denture uses. Descriptive statistics-including frequency, percentage, mean, and standard deviation—were utilized for data analysis.
The findings revealed that the developed service model significantly improved local access to denture services among older adults, reducing clinical service steps from 5 to 3 and shortening treatment duration from 10–12 weeks to 4–6 weeks. Overall satisfaction among the older participants was high to highest, specifically regarding masticatory ability (80.0%), painless denture use during function (84.0%), esthetics (81.3%), and speech pronunciation (68.0%).
In conclusion, implementing this denture service model in sub-district health promoting hospitals effectively enhances access to care, streamlines clinical procedures, reduces treatment duration, and achieves high patient satisfaction. Therefore, this model serves as a feasible framework to improve service efficiency, alleviate hospital overcrowding, and support the development of community-based oral healthcare policies for the elderly.
