Background: Ethical climate in the workplace is a critical factor that promotes ethical behavior among professional nurses in delivering quality patient care, particularly in tertiary hospitals with a structured nursing ethics mechanism. However, empirical evidence reflecting the actual situation in this context remains limited.
Objective: To examine the level of perceived ethical climate in the workplace and the level of ethical behavior among professional nurses in a tertiary hospital with a structured nursing ethics mechanism.
Methods: A cross-sectional descriptive survey was conducted among 406 professional nurses (an 83.20% response rate from a calculated target sample of 488) working across 60 inpatient wards at Srinagarind Hospital, Faculty of Medicine, Khon Kaen University, selected through systematic random sampling. Data were collected using two instruments: the Nurses' Perceived Ethical Climate in the Workplace Questionnaire (54 items; CVI = 0.91, Cronbach's alpha = 0.97) and the Nurses' Ethical Behavior toward Patients Scale (34 items; CVI = 0.93, Cronbach's alpha = 0.98). Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation.
Results: The overall perceived ethical climate in the workplace was at a high level (Mean = 4.25, SD = 0.78). Among the subscales, honesty received the highest mean score (Mean = 4.34, SD = 0.72), while respect for patient autonomy received the lowest score (Mean = 4.14, SD = 0.83). Regarding ethical behavior, the overall score was at a very good level (Mean = 4.56, SD = 0.64). Justice toward patients received the highest mean score (Mean = 4.65, SD = 0.59), while truth-telling received the lowest score (Mean = 4.46, SD = 0.72). At the item level, delivering bad news to patients when they were physically and emotionally ready received the lowest score.
Conclusion: Professional nurses in a tertiary hospital with a systematically structured nursing ethics mechanism reported high levels of perceived ethical climate and ethical behavior. Future continuing education programs should prioritize enhancing nurses' competencies in communicating difficult news to patients and further promoting comprehensive respect for patient autonomy
