Care Management for Pregnant Women Using Amphetamine-Type Stimulants During Labor and Postpartum Period in Thakhantho Hospital, Kalasin Province: Application of Orem's Nursing Theory

Abstract

This multiple case study aimed to examine the effects of applying Orem's Nursing. Theory in caring for pregnant women who use amphetamine-type stimulants during labor and postpartum periods. The study was conducted in the obstetric ward of Thakhantho Hospital, Kalasin Province, from July 10, 2024, to September 30, 2024. The sample consisted of 5 pregnant women who used amphetamine-type stimulants (methamphetamine, crystal methamphetamine, and ecstasy) with complex nursing care challenges during labor and postpartum periods. Based on Orem's Nursing Theory assessment, the findings revealed that during the labor period, patients experienced self-care deficit and health deviation self-care requisites. During the postpartum period, they exhibited dependent-care deficit regarding newborn care and universal self-care requisites deficits in postpartum recovery, such as personal hygiene, food and water intake, elimination, rest, and hazard prevention. Regarding newborn care, deficits were found in areas such as breastfeeding, infant hygiene, abnormal symptom observation, developmental promotion, and self-care deficit. The nursing care plan implemented three systems: supportive-educative nursing system, partly compensatory system, and wholly compensatory system. The care encompassed physical, mental, and social aspects, including coordination for patient and infant safety through drug rehabilitation services and collaborative care networks.
The application of Orem's Nursing Theory in caring for pregnant women using amphetamine type stimulants can be developed further in various aspects: care system development, personnel development, quality of care improvement, family support enhancement, health prevention and promotion, social support monitoring, policy development, research and development, and information system development.
