Development of a Community Care Model for Dependent Older Adults at Phai Subdistrict Health Promoting Hospital, Rattanaburi District, Surin Province

This action research developed a community care model for dependent older adults and examined network participation at Phai Subdistrict Health Promoting Hospital. The PAOR cycle was implemented from March 2025 to March 2026 with 114 network members and
53 dependent older adults. Questionnaires, the Activities of Daily Living (ADL) assessment, and an interview guide had item-objective congruence indices of 0.67–1.00 and Cronbach’s alpha coefficients of 0.82 and 0.75. Descriptive statistics, paired t-tests, and content analysis were used. Among 952 older adults (28.03% of residents), 53 (5.57%) were dependent. The community co-care model integrated an individual database, dependency-specific care plans, family and network roles, home visits, surveillance, monitoring, and reflection. After implementation, network mean scores for knowledge, attitude, and participation increased from 9.25 to 9.66, 4.67 to 4.86, and 4.38 to 4.81. Older adults’ mean health knowledge, attitude and perception of care, and ADL scores increased from 8.49 to 9.12, 4.60 to 4.73, and 12.09 to 15.22. All increases were statistically significant (p < .001). The model links person-level information with active network participation in primary long-term care.
