This mixed-methods study examined self-management and community participation and developed and evaluated a blood pressure control model. Three phases were conducted from March 2025 to March 2026 with 187 hypertensive patients and 179 network members. Questionnaires, blood pressure records, assessments, and interviews had item-objective congruence indices of 0.67–1.00 and Cronbach’s alpha coefficients of 0.79–0.86. Descriptive, correlational, group-comparison, paired-comparison, and content analyses were used. Baseline self-management (M = 3.26, SD = 0.44) and community participation (M = 3.95, SD = 0.71) were moderate. Age correlated negatively with self-management (rₛ = −0.485), whereas duration since diagnosis correlated positively (rₛ = 0.417; p < .001). The seven-component, eight-step model linked assessment, skills, family planning, home visits, community activities, local support, and evaluation. After implementation, mean systolic pressure decreased from 151.62 ± 8.74 to 135.48 ± 7.92 mmHg and diastolic pressure from 91.36 ± 6.28 to 82.14 ± 5.74 mmHg. Self-management increased to 4.18 ± 0.39; all changes were significant (p < .001). Patient and network satisfaction were high (M = 4.30, SD = 0.41 and M = 4.37, SD = 0.43). The model is suitable for continuous community-based primary care.
