The effect of a counseling program on knowledge and decision-making regarding renal replacement therapy in patients with chronic kidney disease (stage 4-5) at Maesai hospital, Chiangrai province

This quasi-experimental study aimed to examine the effects of a counseling program on knowledge and decision-making regarding renal replacement therapy among patients before and after receiving the program. The sample consisted of 30 patients with stage 4–5 chronic kidney disease who had not yet received renal replacement therapy and were attending a nephrology clinic at Mae Sai Hospital, Chiang Rai Province.Participants were selected using purposive sampling.The intervention was a counseling program developed based on the concepts of shared care and shared decision-making. Data collection instruments included a demographic questionnaire, a knowledge test on chronic kidney disease and renal replacement therapy, and a decision-making questionnaire regarding renal replacement therapy options. Data were analyzed using descriptive statistics, Fisher's exact test, and one-way repeated measures ANOVA.
The results revealed that: 1) after receiving the counseling program, the mean knowledge scores immediately and at 1-month follow-up were at a moderate level and significantly higher than those before the intervention (p < .001); and 2) the proportion of patients who made decisions regarding renal replacement therapy significantly increased (p < .05), with most patients choosing hemodialysis.In conclusion, the counseling program was effective in improving knowledge and promoting decision-making regarding renal replacement therapy among patients with stage 4–5 chronic kidney disease. The program can be applied as a guideline for improving counseling services in nephrology clinics.
