Research and Development Model of Self-health carefulness with Medical Promotion in Non-Communicating Diseases (NCDs) older patient in Khaoya subdistrict, Sribunpot district, Phatthalung province

The aim of this participatory action research is to develop a model of self-health carefulness and medical support mixed among elderly people with non-communicating disease (NCDs). Kemmis and McTaggart’s action research spiral and chronic care model Wag of Wagner et. al. were mixed to this conceptual framework. Model was start to development and data were collected on Jan 2024, 25 in time use 4 month from 30 participants in the total 194 target population who were selected by include and exclude criteria with 18 tools in 3 types which passed the quality inspection according to the type of tool. Content analysis was used to quality data and descriptive statistic was used to quantity data and finding were presented by model plot with descriptive words. The results found that
