Efficacy of Selective Nerve Root Block versus Epidural Steroid Injection for Treatment in Patient with Lumbar Radicular Pain

รูปแบบการวิจัย: การวิจัยเชิงทดลองแบบศึกษาย้อนหลัง (retrospective interventional study)
วัตถุประสงค์: เพื่อเปรียบเทียบวิธีการฉีดยาสเตียรอยด์บริเวณช่องกระดูกสันหลังสองวิธีในการรักษาผู้ป่วยที่มีอาการปวดหลังร้าวลงขา
วิธีการวิจัย: การวิจัยทำในผู้ป่วยที่มีอาการปวดหลังร้าวลงขาที่ได้รับการฉีดยาบริเวณรากประสาท 39 คน และบริเวณช่องกระดูกสันหลัง 28 คน โดยวัดผลจากระดับความปวด (Visual analog scale : VAS) และแบบ สอบถามออสเวสทรี รุ่น 1.0 ฉบับภาษาไทยในผู้ป่วยปวดหลัง (Oswestry Disability Index : ODI) ก่อนฉีดยา, 2 สัปดาห์, 6 สัปดาห์ และ 3 เดือน หลังฉีดยา ตามลำดับ
ผลการวิจัย: ค่าเฉลี่ยและค่าเบี่ยงเบนมาตรฐานของระดับความปวดเปรียบเทียบกัน ระหว่างกลุ่มที่ได้รับการฉีดยาบริเวณรากประสาทและในกลุ่มที่ได้รับการฉีดยาบริเวณช่องกระดูกสันหลังลดลงจาก 5.69 1.49 เหลือ 3.15 t 1.39 และ 6.29 t 2.0 เหลือ 3.5 1.48 ตามลำดับ (p=0.12) ในขณะที่ค่าเฉลี่ยและค่าเบี่ยงเบนมาตรฐานของ ODI ระหว่างกลุ่มที่ได้รับการฉีดยาบริเวณรากประสาทและในกลุ่มที่ได้รับการฉีดยาบริเวณช่องกระดูกสันหลังลดลงจาก 43.1 + 16.8 เหลือ 26.4 t 14.0 และ 49.4 17.7 เหลือ 32.9 + 14.3 ตามลำดับ (p=0.019)
สรุปผล: การฉีดยาสเตียรอยด์บริเวณกระดูกสันหลังส่วนเอวแบบฉีดยาบริเวณรากประสาท เหนือกว่าการฉีดยาบริเวณช่องกระดูกสันหลังในแง่ของคุณภาพชีวิตหลังการรักษาที่ดีขึ้น แต่ในแง่ของระดับความปวดพบว่าไม่แตกต่างกันทั้งสองวิธี
