ความก้าวหน้าในการรักษา Diffuse Intrinsic Pontine Glioma (DIPG): กลยุทธ์และนวัตกรรมใหม่

Diffuse intrinsic pontine glioma (DIPG) เป็นเนื้องอกสมองในเด็กที่มีความรุนแรงสูงและมีพยากรณ์โรคไม่ดี แม้ว่าความก้าวหน้าทางชีววิทยาระดับโมเลกุลจะช่วยให้เข้าใจกลไกการเกิดโรคมากขึ้น แต่ยังไม่มีวิธีรักษาที่สามารถทำให้หายขาดได้ บทความนี้มีวัตถุประสงค์เพื่อทบทวนองค์ความรู้ปัจจุบันเกี่ยวกับพยาธิกำเนิดระดับโมเลกุล การวินิจฉัยการรักษามาตรฐาน และแนวทางการรักษาใหม่ของ DIPG โดยพบว่าความผิดปกติของ H3K27 ได้แก่ H3.3K27M, H3.1K27M และ EZHIP ร่วมกับการกลายพันธุ์ของ TP53, PDGFRA และ ACVR1 มีบทบาทสำคัญต่อการเกิดโรค และการดื้อต่อการรักษาการตรวจคลื่นแม่เหล็กไฟฟ้ายังคงเป็นเครื่องมือหลักในการวินิจฉัย ขณะที่การตัดชิ้นเนื้อแบบ stereotactic biopsy มีบทบาทเพิ่มขึ้นในการวิเคราะห์ระดับโมเลกุล การฉายรังสียังคงเป็นการรักษามาตรฐาน แต่ประโยชน์ของยา temozolomide ต่อการเพิ่มอัตราการรอดชีวิตยังมีจำกัด ปัจจุบันมีการพัฒนาแนวทางการรักษาใหม่ ได้แก่ CAR-T cell therapy, oncolytic virotherapy, วัคซีน SurVaxM และ immune checkpoint inhibitors ซึ่งให้ผลเบื้องต้นที่น่าสนใจ อย่างไรก็ตาม ยังจำเป็นต้องมีการศึกษาทางคลินิกและการวิจัยเพิ่มเติมเพื่อยืนยันประสิทธิภาพ ความปลอดภัย และพัฒนาการรักษาแบบจำเพาะที่สามารถเพิ่มผลลัพธ์ทางคลินิกของผู้ป่วย DIPG ในอนาคต
