ภาวะเลือดไปเลี้ยงสมองมากเกินหลังการผ่าตัด ต่อหลอดเลือดสมองในผู้ป่วยหลอดเลือดสมองโมยาโมยา

Moyamoya disease (MMD) คือภาวะที่มีการตีบหรือตันของหลอดเลือดแดง internal carotid artery (ICA) ที่บริเวณส่วนปลาย หรือ middle cerebral artery (MCA) หรือ anterior cerebral artery (ACA) ซึ่งทำให้ผู้ป่วย มีอาการได้ทั้งภาวะสมองขาดเลือด (cerebral ischemia) หรือ ภาวะเลือดออกในช่องกะโหลกศีรษะ (intracranial hemorrhage) การรักษาที่เป็นมาตรฐานในปัจจุบันคือการต่อหลอดเลือดสมอง (direct bypass) เชื่อมระหว่าง superficial temporal artery (STA) และ middle cerebral artery (MCA) ร่วมกับการทำ indirect bypass ซึ่ง ภาวะแทรกซ้อนที่สำคัญของการทำ direct bypass คือภาวะเลือดไปเลี้ยงสมองมากเกิน (cerebral hyperperfusion syndrome; CHS) ทำให้มีอาการหลังการผ่าตัดได้ตั้งแต่ ปวดศีรษะ, ภาวะชัก, ความผิดปกติทางระบบประสาท, หรือ ภาวะความรู้สึกตัวผิดปกติ โดยปัจจุบันยังไม่มีแนวทางการจัดการภาวะ CHS ที่ชัดเจนทั้งการวินิจฉัยและการรักษา ทางผู้เขียนและคณะจึงได้รวบรวมลักษณะอาการ, ปัจจัยเสี่ยง, แนวทางการวินิจฉัย, การรักษา, และการป้องกันเอาไว้ ในบทความฉบับนี้
