ภาวะแทรกซ้อนของการคลอดบุตรก่อนมาถึงโรงพยาบาล ณ โรงพยาบาลสมุทรสาคร

วัตถุประสงค์ : เพื่อศึกษาภาวะแทรกซ้อนของการคลอดบุตรก่อนถึงโรงพยาบาล (Born Before Arrival: BBA)
วิธีดำเนินการวิจัย : เป็นการศึกษาย้อนหลัง case control study ใช้ข้อมูลจากมารดาและทารก จำนวน 354 ราย แบ่งเป็นกลุ่มคลอดในโรงพยาบาล 177 ราย และกลุ่มคลอดก่อนถึงโรงพยาบาล 177 ราย วิเคราะห์สถิติโดยใช้ลอจิสติกถดถอย (Logistic Regression)
ผลการวิจัย : กลุ่ม BBA ส่วนใหญ่มาโรงพยาบาลด้วยระบบบริการการแพทย์ฉุกเฉิน ร้อยละ 64.4 เป็นการคลอดที่บ้านร้อยละ 70.06 มารดาที่มีสัญชาติพม่ามีความเสี่ยงในการคลอด ก่อนถึงโรงพยาบาลสูงกว่ามารดาชาวไทยอย่างมีนัยสำคัญ (Adj. OR = 1.94, p < 0.01) กลุ่ม BBA พบภาวะแทรกซ้อนของมารดา จำนวน 9 ราย (ร้อยละ 5.08) ภาวะแทรกซ้อนในทารก 127 ราย (ร้อยละ 71.75) ซึ่งภาวะแทรกซ้อนที่มีนัยสำคัญ ได้แก่ ภาวะติดเชื้อ (Neonatal sepsis) 29 ราย (ร้อยละ 16.38) ภาวะเลือดข้น (Polycythemia) 22 ราย (ร้อยละ 12.43) ภาวะอุณหภูมิกายต่ำ (Hypothermia) 100 ราย (ร้อยละ 56.50) และทารกจำนวน 12 ราย (ร้อยละ 6.78) ต้องเข้ารับ การรักษาในหอผู้ป่วยหนักสำหรับทารกแรกเกิด (NICU) เมื่อวิเคราะห์ปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะแทรกซ้อนในทารกอย่างมีนัยสำคัญ คือ กลุ่ม BBA (Adj. OR = 4.91, p < 0.01) และมารดาที่มีโรคประจำตัว (Adj. OR = 3.14, p 0.02)
สรุปผล : กลุ่มคลอดก่อนถึงโรงพยาบาล (BBA) มีความเสี่ยงสูงต่อการเกิดภาวะแทรกซ้อนในทารก โดยเฉพาะภาวะติดเชื้อ ภาวะเลือดข้น และภาวะอุณหภูมิกายต่ำ ส่งผลให้บางรายต้องเข้ารับการรักษาในหอผู้ป่วยหนักสำหรับทารกแรกเกิด (NICU)
