Dynamics and Constraints of the 'Addict as Patient' Policy: An Analysis of Recidivism through the Lens of Frontline Practitioners

This research aims to analyze the dynamics of repeated drug use and explore the limitations and policy gaps of the "addicts are patients" policy (Narcotics Code, B.E. 2564) in managing psychiatric drug patients at the local level. Data were collected through in-depth interviews with four key informant groups: legal professionals, public health officials, community leaders, and families of repeat offenders in Wang Hin District, Si Sa Ket Province. Content analysis was guided by 13 behavioral science and sociological theories. The findings reveal that current recidivism behavior has transitioned into a state of "psychiatric drug patients" with a significant risk of causing violence. An analysis of frontline limitations uncovered that: 1) the legal sector faces inconsistencies between the policy's intent and arrest indicators, alongside a lack of authority to detain high-risk individuals; 2) the public health sector suffers from overwhelming workloads and insufficient beds, providing only short-term palliative care; 3) the community lacks budget, safety tools, and harbors stigmatizing attitudes; and 4) families bear risks to the point of structural collapse. These factors create a "revolving door" phenomenon where users cycle in and out of the treatment system without being healed. Furthermore, a critical gap identified is a "broken chain of communication" among courts, hospitals, and communities, driven by the absence of a core host to jointly manage risk data. Policy recommendations suggest structural reforms by:
1) developing a centralized case management system via a unified digital platform;
2) establishing district-level, high-security, semi-compulsory treatment centers to isolate patients from communities; and 3) implementing compulsory treatment combined with criminal measures for individuals with violent behaviors to restore public safety based on utilitarian principles.
