Mass casualty accident investigation: a collision between the pickup truck, the SUV and the motorcycle in Kuchinarai district, Kalasin Province, Thailand, 25 February 2023

Background: On 25th February 2023, Office of Disease Prevention and Control Region 7th, Khon Kaen province, Thailand (ODPC7) received the notification from Kalasin Provincial Public Health Office (KPPHO) about the mass casualty accident on the road no. 299, Kuchinarai District, Kalasin Province, Thailand. There were 15 injured and 4 death cases. Therefore, ODPC7 cooperated with KPPHO, Kuchinarai Crown Prince Hospital and networks to investigate the event on 27th February 2023. The objectives were to confirm the diagnosis, to describe the epidemiological characteristics, to identify the causes/risk factors, and to recommend the prevention and control measures.
Methods: The descriptive epidemiologic study was performed by reviewing the medical records, interviewing patients, healthcare personnel, rescuers, and networks, conducting the vehicles and environmental survey, and identifying the causes/risk factors of the accident/injury by Haddon Matrix Model.
Results: For this accident, there were 9 mild injured cases, 2 moderate injured cases, 4 severe injured cases and 4 death cases. The accident occurred according to the misestimation of the pickup truck driver about the distance from the front car and the vehicle’s speed he used. The risk factors of injury/death were age ≥ 60 years old, undertake the front car, not use of seatbelt, travelling unrestrained on the back of a truck, seat location, close to the crashing point, not wear a crash helmet, many passengers on motorcycle, airbag disfunction, no speed limit sign, no semi–rigid guardrail, and roadside ravines.
Recommendations: We recommend 1) Drivers and occupants always wear the seatbelts/crash helmets and follow the traffic laws when travelling by any vehicle. 2) The car owners should take their cars checkup regularly. 3) Policemen strictly enforce the traffic laws regarding undertake the front car, travelling on the back of a truck and use of seatbelt. 4) The Highway District establish the speed limit signs/semi–rigid guardrails. and 5) Public health agencies and networks communicate the risk regarding road safety issue to the community.
