Abstract

Background: Dengue infection is one of important public health problem in Thailand and Bangkok. The incidence rate of dengue in Bangkok in 2016 was 131.35 per 100,000 populations. This study aimed to determine characteristics of quantitative and qualitative attributes of Dengue surveillance system of one private hospital in Bangkok, and study of the flow of reporting system.
Methods: Study of the quantitative characteristics by gathering data from cases reported in national surveillance system (R506), medical records with doctor diagnosis by defined ICD-10-TM from January 1, 2016 to December 31, 2016. Quantitative data was analyzed by sensitivity and predictive value positive. Study qualitative data by interviewing each responsible person in the surveillance system and summarized the key content.
Results: Of the 313 medical records were reviewed, 126 cases were met the dengue definition and 29 cases were reported in the national surveillance system. The sensitivity was 23% while predictive value positive (PVP) was 74.4%. Reporting data can represent a patient gender, age and time of illness. Timeliness of case reporting within 24 hours was 40.45%, 1 0 0 % complete. The accuracy of the name data was 56.4%. For qualitative study, it was found that most of the personnel accepted the surveillance system. The surveillance had simplicity, flexibility, stability and usefulness.
Conclusions: Dengue surveillance system evaluation of one private hospital in Bangkok had result of low sensitivity and inaccurate information. These result caused by lack of re-checking the report and data collection is paperbased. However, this reporting system had good predictive value positive, representativeness, completeness and timeliness. Qualitative data found that personnel accepted the surveillance system. Improvement and development of surveillance systems may be achieved through the development of a system for monitoring whether any patient has not reported the disease, developing a database from the hospital database, and linking it to the reported system. In addition, support personnel related to the surveillance system to continued training.
