Abstract

Backgrounds: Recently, Bureau of Occupational and Environmental Diseases analyzed the situation of acute toxic effects from pesticide poisoning from 43-folder reporting system (HDC service Health Data Center: HDC). Due to concern of data quality, an evaluation of the reporting system was performed in Nong Bua Lum Phu province. The study aims to examine reporting procedures of toxic effects of pesticides in 5 selected hospitals, to evaluate quantitative and qualitative attributes and to provide recommendations for improving data quality.
Methods: We reviewed medical records of patients receiving medical treatment in the study hospitals during January 1–December 31, 2017. A case with toxic effect of pesticides is defined as a patient having history of exposure, and subsequently, developing any symptoms related to toxic effects of pesticides. However, any patients getting sick resulting from pesticides poisoning were excluded. Criteria for reporting is the patient on HDC with the ICD-10 coding as T60.0-T60.9 for the principal diagnosis and as X48 for the external cause. We interviewed 25 people as key stakeholders for qualitative study and reviewed 126 medical records for quantitative study.
Results: Of 126, 101 medical records met case definition. Sensitivity of reporting ranged between 0% and 100%; 2 district hospitals were found zero reporting. However, positive predictive value (PPV) were found to be 100% in 2 study hospitals, 50% in 1 hospital, while PVP from the 2 district hospitals cannot be evaluated. According to qualitative attributes, simplicity was fair, but ICD-10 coding was somewhat complicated. The system revealed good acceptability and stability although the policy could be changed if the chief executive level changed.
Conclusions and discussions: The wide range of the sensitivity might be caused by an error in diagnosis and ICD- 10 coding. Therefore, implementation of guideline for history taking, diagnosis and ICD-10 coding is necessary for improving data quality for surveillance.
