AFP surveillance is a key strategy of the global polio eradication program. Completeness of AFP case reports is an important component leading to rapid case investigation and timely disease control. Evaluation of AFP surveillance or AFP active case search is conducted to check the completeness of AFP case reports in hospitals as reporting sites by periodical reviews of medical records of patient under 15 years. In the year 2012, result of AFP active case search in 59 hospitals from 15 provinces showed 37 missing AFP cases. As well as 43 missing AFP cases of 62 hospitals from 25 provinces were shown in the year 2013. The major causes of missing AFP case report are misunderstanding of some medical personnel that it is not necessary to report the AFP cases having definite diagnosis of other diseases which are not suspected polio and lacking of knowledge and experience to AFP surveillance system. To improve the completeness of case report, AFP surveillance and principle of global polio eradication program should be emphasized to medical personnel in hospitals.
