Abstract

วันที่ ๑ สิงหาคม 2550 สำนักระบาดวิทยาได้รับรายงานการระบาดซ้ำของเชื้อบิด Shigella sumci ในโรงเรียนเอกชนแห่งหนึ่ง จึงเข้าดำเนินการสอบสวนโรคระหว่างวันที่ 10 - 17 สิงหาคม 2550 เพื่อค้นหาสาเหตุของการระบาค และป้องกันควบกุมโรคต่อไป
ทำการศึกษาโดขใช้วิธีทั้งระบาดวิทยาเชิงพรรณนาและเชิงวิเคราะห์ โดยศึกษาข้อมูลในระบบเฝ้าระวังทบทวนเวชระเบียนของผู้ป่วย และค้นหาผู้ป่าขรายใหม่โดยให้นิยามผู้ป่วยว่า นักเรียน ครู หรือเจ้าหน้าที่ของโรงเรียน ที่มีอาการถ่ายเหลวอย่างน้อย 3 ครั้งต่อวัน หรือถ่ายเป็นน้ำ หรือถ่ายเป็นมูกอย่างน้อย 1 ครั้งต่อวัน ในระหว่างวันที่ 1-17 ส.ค. 2550 ส่วนการศึกษาระบาดวิทยาเชิงวิเคระห์ทำเฉพาะในนักเรียน ครู และเจ้าหน้าที่ อื่นๆ ที่เข้าค่ายคุณธรรมจริยธรรม ซึ่งจัดในวัดแห่งหนึ่งบริเวณใกล้โรงเรียน ระหว่างวันที่ 3 - 4 สิงหาคม 2550 โดยวิธี Retrospective Cohort Study (นักเรียนที่ไปเข้าค่ายเป็นนักเรียนเฉพาะชั้น ป.๑) วิเคราะห์ทางสถิติโดยใช้โปรแกรม STATA version 10.0 เพื่อวิเคราะห์ค่า Relative risk 95% confidence iterval, Adjusted odd ratio โดยวิธี Multiple logistic regression และทดสอบ Dose response relationshijp ของอาหารและน้ำดื่มที่สงสัยทำ Rectal swab cultures (RSC) ในผู้ป่วยและคนทำครัว สำรวจครัว และศึกษาวิธีการประกอบอาหาร ตลอดจนส่งน้ำดื่มของโรงเรียนเพื่อตรวจทางห้องปฏิบัติการ
