Coverage of Diabetic Retinopathy Screening and Prevalence of Diabetic Retinopathy in Sisaket Province 2010 - 2012

Abstract

Background: In 2000, WHO reported that the global diabetic mellitus is 171 million, and estimated that to increase to 366 million by the year 2030. Prevalence of diabetic mellitus in Thailand increased from 2.3% in 1991 to 6.9% in 2009, and 15 years after diagnosis, 2% developed blindness and 10% developed some degree of vision problem. In order to realize the problems and their magnitudes in Sisaket province, this study reports the prevalence of diabetic mellitus, the coverage of diabetic retinopathy screening and the prevalence of diabetic retinopathy in Sisaket from 2010 to 2012, and acknowledges the problems in some districts of which the coverage of screening is less than 50%.
Methodology and statistics: Retrospective study and descriptive statistics.
Result: The prevalence of diabetes in Sisaket province from 2010 to 2012 was 1.95, 2.23 and 2.21, respectively. In 2010, the coverage of screening vision not specific to retinopathy was 79.46%. The coverage of diabetic retinopathy screening in 2011 and 2012 was 74.80% (23,387cases) and 66.01% (21,220 cases). The prevalence of diabetic retinopathy was 2.16% (676 cases) in 2011, and 2.97% (538 cases) in 2012, classified as NPDR 2.75% and PDR 0.22%. For the problem of the districts with less than 50% coverage was the wide responsible areas with inconvenient access to the hospitals.
Conclusion and discussion: The prevalence of diabetes in Sisaket province from 2010 to 2012 was 1.95, 2.23 and 2.21 which was quite low when compared to the national prevalence, and comparable to that from 20 years ago. The prevalence of diabetic retinopathy in this study was classified as NPDR 2.75% and PDR 0.22% which is five to ten times lower than when compared with others study provinces with NPDR 12.00 - 27.27% and PDR 1.20 - 5.20%. This can be explained by the high screening coverage of 66.01 - 74.80%. A solution to the problem of less than 50% coverage areas is to emphasize the proactive mobile services.
