Abstract

Background: Pulmonary tuberculosis remains a significant public health concern in Buriram Province and Chalermphrakiat District. No formal evaluation of the tuberculosis surveillance system at Chalermphrakiat Hospital had previously been conducted, and deficiencies in data completeness and reporting had been identified. This study aimed to assess surveillance reporting practices, evaluate both qualitative and quantitative attributes of the system, and propose recommendations for improving surveillance efficiency.

Methods: A cross-sectional descriptive study was conducted to evaluate the quantitative and qualitative characteristics of the surveillance system. The study population included all patients diagnosed with pulmonary tuberculosis or related ICD codes who received treatment at Chalermphrakiat Hospital between October 1, 2022, and September 30, 2025. Data were obtained from outpatient and inpatient medical records via the HosXP and NTIP programs. Descriptive statistics were applied for quantitative analysis, and content analysis was used for qualitative findings.

Results: A review of 4,817 medical records identified 77 patients meeting the case definition for pulmonary tuberculosis, of whom 68 were reported through the NTIP program, yielding a sensitivity of 88.31%. All 68 reported cases met the surveillance case definition, corresponding to a positive predictive value of 100%. Data quality for key variables—age, sex, address at time of illness, and date of diagnosis—was 100% accurate. Timeliness was 82.35%. The surveillance system demonstrated adequate representativeness. Simplicity, acceptability, stability, flexibility, and utility of the NTIP-based system were rated favorably by all relevant personnel.

Discussion and Recommendations: The pulmonary tuberculosis surveillance system at Chalermphrakiat Hospital performed at a satisfactory level and demonstrated overall effectiveness. To further strengthen the system, epidemiological staff should conduct thorough case screening in accordance with the surveillance case definition, verify medical record data for completeness and accuracy prior to entry into NTIP, and ensure that newly diagnosed cases are registered within seven days of diagnosis.
