บทนำ: ผู้ป่วยโรคไตเรื้อรังระยะท้ายเผชิญกับความต้องการการดูแลที่ซับซ้อนในทุกมิติ ทั้งด้านร่างกาย จิตใจ สังคม และจิตวิญญาณ แต่ระบบบริการสุขภาพระดับชุมชนยังขาดการบูรณาการการวางแผนการดูแลล่วงหน้า และการดูแลแบบประคับประคองอย่างเป็นระบบ ส่งผลให้การดูแลในระยะท้ายไม่สอดคล้องกับคุณค่าและความเจตจำนงของผู้ป่วย
วัตถุประสงค์: เพื่อนำเสนอองค์ความรู้เกี่ยวกับการพัฒนารูปแบบการบูรณาการการวางแผนการดูแลล่วงหน้าเพื่อส่งเสริมการดูแลแบบประคับประคองในผู้ป่วยโรคไตเรื้อรังระดับชุมชน
สาระสำคัญ: การพัฒนารูปแบบการบูรณาการการวางแผนการดูแลล่วงหน้าเพื่อส่งเสริมการดูแลแบบประคับประคองในผู้ป่วยโรคไตเรื้อรังระดับชุมชน ประกอบด้วย 4 ขั้นตอนหลัก ได้แก่ การวิเคราะห์สถานการณ์ การออกแบบรูปแบบ การทดลองใช้ และการปรับปรุงรูปแบบ โดยมีองค์ประกอบหลัก 6 องค์ประกอบ คือ 1) การมีภาวะผู้นำ 2) การเสริมศักยภาพ 3) กระบวนการดูแล 4) ระบบข้อมูลและสารสนเทศ 5) การมีส่วนร่วมของชุมชน และ 6) เครื่องมือสนับสนุนการดำเนินงาน ซึ่งผลลัพธ์ของการดำเนินงานตามรูปแบบส่งผลต่อผู้ป่วย ต่อครอบครัว ต่อบุคลากรสุขภาพและชุมชน และต่อระบบบริการสุขภาพ
สรุป: รูปแบบการบูรณาการการวางแผนการดูแลล่วงหน้าเพื่อส่งเสริมการดูแลแบบประคับประคองในผู้ป่วยโรคไตเรื้อรังระดับชุมชน เป็นแนวทางการดูแลเชิงระบบที่ยึดผู้ป่วยเป็นศูนย์กลางร่วมกับการมีส่วนร่วมของชุมชน เพื่อยกระดับคุณภาพชีวิต ลดภาระของครอบครัว และเพิ่มประสิทธิภาพของระบบบริการสุขภาพ
ข้อเสนอแนะ: ควรผลักดันการนำรูปแบบนี้ไปปฏิบัติในหน่วยบริการปฐมภูมิทั่วประเทศ พร้อมทั้งประเมินผลลัพธ์ในระยะยาวเพื่อใช้เป็นฐานข้อมูลเชิงนโยบายในการพัฒนาระบบการดูแลแบบประคับประคองในผู้ป่วยโรคไตเรื้อรังระดับชุมชนอย่างยั่งยืน
