The Development of a Method for Patient Preparation for Intravitreal Injections Using the “Mark Site” Innovation and Alert Tags at Maharaj Nakhon Si Thammarat Hospital

This research and development study aimed to: (1) examine the situation, problems, and needs related to patient preparation for intravitreal injections; (2) develop a patient preparation method using the Mark site innovation and Alert tags; and (3) evaluate the effectiveness of the Mark site innovation and Alert tags among patients undergoing intravitreal injections at Maharaj Nakhon Si Thammarat Hospital. The study was conducted in three phases. Phase 1 involved an assessment of the situation, problems, and patient needs using qualitative methods and document analysis. Phase 2 focused on the development of the Mark site innovation and Alert tags. Phase 3 evaluated the effectiveness of the developed innovations. Data were analyzed using descriptive statistics, the Wilcoxon signed-rank test, and the one-sample t-test.
The results indicated that: (1) the incidence of near-miss events decreased in 2023 compared to 2022, with no incidents reported in 2024; however, a rate of 1.92% was observed in 2025. Physicians, registered nurses, and nurse assistants emphasized the importance of the characteristics of the Mark site and Alert tags. (2) The Mark site and Alert tag innovations consisted of: (a) color coding to indicate the type of procedure (red for right eye injection, yellow for left eye injection, and pink for bilateral injections); (b) tag size (9 × 6 cm); and (c) patient identification details (full name and age) printed in Angsana New font, size 80, bold. (3) The mean satisfaction scores before and after implementation were significantly different (p = 0.001). The mean score for correct practice in preventing wrong-patient, wrong-site, and wrong-procedure intravitreal injections was 9.47, which was significantly higher than the predefined criterion (90%) (p < 0.001). No near-miss incidents were reported after implementation. The developed Mark site and Alert tag innovations are practical for clinical use in surgical preparation.
Therefore, they should be implemented in other ophthalmology procedures to reduce adverse incidents and be established as a policy within operating rooms.
