The Implementation of Model Development of Integrated Primary Care in Health Improvement for Older People with Intrinsic Capacity Decline.

Abstract

Healthy ageing as the process of developing and maintaining the functional ability and intrinsic capacity that enables wellbeing in older age. The purpose of implementation research were knowledge translated to guideline of integrated care practices and capacity building of primary care team. The research were operation in 103 of Sub-district Health Promotion Hospital and Primary Care Unit with mixed-method which were document , qualitative and quantitative research in August 2022 to May 2024.

The result show that implementation Model were 4 step as (1) Evidence review for practices guideline development (2) Health care team capacity building (3) Health care integrated process redesign and (4) Evaluation capacity building. The model of care included 2 elements as (1) Integrate Health and Social Care Services as initiating implementation stage (mean = 18.82 for 26 scores) and (2) Align Care Systems to Support Integrated Care as  initiating implementation stage (mean = 16.04 for 26 scores). The findings of facilitation factors as transformative leadership , policy and value of outcomes of goals implementation and co-production project in operational context. The factors affected to health outcomes as 3 domain intrinsic declined, activities of diary living (ADL) and life satisfaction and quality of live of older person with significant level (p-value = 0.05)

The results informed Department of MOPH, Provincial Administration Organization (PAOs) should to develop and implementation policy and program on integrated care for older people. Including ICTs develop and appropriately health intervention measurement on integrated and self-care and self-management.
