Abstract

This article aims to theoretically synthesize strategic human capital development in Thai healthcare organizations and explain its relationship with long-term health system sustainability. A conceptual and integrative literature review was conducted using Scopus, PubMed, and Google Scholar, covering literature published between 1991 and 2024. Literature was selected based on its relevance to Human Capital Theory, Strategic Human Resource Management (SHRM), the Resource-Based View (RBV), health workforce development, and health system sustainability in international and Thai contexts, with consideration of its alignment with the objectives and conceptual framework of the article. The analytical process involved categorizing key concepts and themes, comparing and linking theoretical perspectives and findings, examining conceptual relationships, and synthesizing common elements to develop an integrated theoretical framework. The synthesis resulted in the Strategic Human Capital Development for Thai Health System Sustainability Model (SHCD-TH Model), which proposes a multilevel mechanism linking strategic workforce foresight, competency-based development, integrated human resource governance, and a learning and innovation ecosystem to organizational capability and health system sustainability outcomes. These outcomes include fiscal sustainability, quality and patient safety, equity in access, and systemic resilience. The synthesis indicates that strategic human capital development should not be regarded merely as an operational human resource function but as a strategic infrastructure connecting individual capabilities, organizational capacity, and health system outcomes. The SHCD-TH Model provides an integrated theoretical perspective for understanding how strategically aligned human capital development can strengthen organizational capabilities and contribute to the long-term sustainability of the Thai health system. The findings have implications for health workforce planning, human resource governance, competency development, and health system policy.
