A case investigation of Streptococcus suis infection in a patient with total knee arthroplasty, Thepharak District, Nakhon Ratchasima Province, Thailand, August 2018

Background: Septic arthritis caused by Streptococcus suis ( S. suis) has rarely been reported in Thailand. We reported S. suis septic arthritis in a female patient with previous total knee arthroplasty of both legs in Thepharak District, Nakhon Ratchasima Province. The objectives were to verify diagnosis, determine epidemiological characteristics of the case, find the source and transmission of the disease, and recommend control measures.
Methods: The investigation included reviewing medical records of the patient, interviewing the patient and her relatives, and environmental investigation in the affected area. The patient had history of visiting her younger sister who owned a pig farm in Lop Buri Province and had Streptococcal meningitis during her visit in April 2018. A medical record of her sister was also reviewed.
Results: This female patient was 62 years old and had hypertension and dyslipidemia. She received total knee arthroplasty of right leg in 2013 and left leg in 2015 at Nakhon Ratchasima Regional Hospital. On 22 April 2018, she went to follow-up her arthroplasty. She had arthralgia and difficult walk and was diagnosed of septic arthritis of both knees. Alpha-hemolytic Streptococci were isolated from her knee fluid aspiration. She received debridement and exchange of arthroplasty of both knees. S. suis serotype 2 was isolated from her joint fluid specimens. She recovered after 47 days of treatment in the hospital. On 14 April 2018, she visited her sister and helped in resection of a dead pig. Her sister who owned a pig farm developed meningitis in late March 2018. Streptococci were isolated from her cerebrospinal fluid and blood.
Conclusions: Septic arthritis of both knees caused by S. suis serotype 2 was confirmed in this patient with previous total knee arthroplasty. The patient might be infected by contact exposure to the organisms in the pig farm in Lop Buri Province.
