Abstract

Non-communicable diseases (NCDs) are a major global public health problem and are closely associated with unhealthy behaviors, leading to overweight and obesity—key risk factors for NCDs. Adolescence is a critical period for the establishment of long-term health behaviors. Health literacy (HL) plays an important role in shaping these behaviors; however, evidence on the association between HL and overweight/obesity among Thai adolescents remains limited. The objectives were to evaluate the association between HL and overweight/obesity, and to determine the level of HL, the prevalence of overweight and obesity, and factors associated with HL among secondary school students. An analytical cross-sectional study was conducted among secondary school students. The sample size was calculated using Cochran’s formula (n = 385), with proportionate stratified sampling by grade level. Data were collected using a self-administered HL questionnaire. Associations between HL and body mass index were analyzed using the Kruskal–Wallis test and Spearman’s rank correlation, and multivariable logistic regression was used to identify factors associated with HL.

The results indicated that a total of 417 students participated in the study. The prevalence of overweight and obesity was 27.8%. HL levels were categorized as insufficient (4.32%), adequate (46.28%), and excellent (49.40%). No statistically significant association was found between HL and BMI using both the Kruskal–Wallis test and Spearman’s rank correlation. Multivariable analysis showed that students in upper secondary school were more likely to have higher HL than those in lower secondary school (Adjusted Odds Ratio [AOR] = 2.36, 95% Confidence Interval [95%CI]: 1.47–3.80). Late adolescents were also more likely to have higher HL than early adolescents (AOR = 2.74, 95% CI: 1.31–5.72). In contrast, students whose parents worked in trade, private business, or manual labor were less likely to have higher HL (AOR = 0.58, 95% CI: 0.36–0.93).

The findings may be useful for developing school-based health promotion policies. School-based interventions should integrate HL development with behavioral and environmental strategies to support long-term NCD prevention.
