Experiences and Roles of Village Health Volunteers in Caring for Patients with Type 2 Diabetes Mellitus in a Community in Ranong Province: A Descriptive Qualitative Study

Abstract
This descriptive qualitative study aimed to explore the experiences and roles of village health volunteers in caring for patients with type 2 diabetes mellitus in a community in Ranong Province, Thailand. Purposive sampling was used to recruit 25 key informants, including 15 village health volunteers and 10 patients with type 2 diabetes mellitus. Data were collected through in-depth interviews and focus group discussions and analyzed using inductive qualitative content analysis.
The findings revealed that village health volunteers functioned as frontline community caregivers by providing continuous follow-up, home visits, health education, symptom monitoring, and referral coordination for patients with type 2 diabetes mellitus. Their caregiving roles were performed under limitations related to disease-specific knowledge, symptom assessment skills, available resources, and healthcare system support. Although village health volunteers received support from community members, families, and healthcare teams, they continued to face challenges associated with patients’ health behaviors and systemic constraints. As a result, diabetes care in the community relied heavily on personal experience, experiential learning, and community collaboration to sustain ongoing patient support.
In conclusion, the experiences of village health volunteers reflect their important role as a linkage mechanism between the community and the healthcare system in the management of type 2 diabetes mellitus, despite limitations in competency and resources. Strengthening the capacity of village health volunteers alongside enhancing support systems within primary healthcare settings may contribute to more continuous and sustainable diabetes care in the community.
