Nursing care for patient with head and chest injuries accompanied by pneumothorax

Introduction: Concomitant severe traumatic brain injury and thoracic trauma with pneumothorax is a life-threatening crisis. Brain dysfunction combined with restricted lung expansion leads to severe hypoxia and multi-organ failure, requiring urgent intervention.
Objective: To study safe, professional nursing care for a patient with combined head and chest injuries and pneumothorax, minimizing complications from pre-hospital care to tertiary center transfer.
Method: A case-study design of one patient was utilized. The nursing process integrated Holistic Care and Orem’s Self-Care Deficit Theory, implementing a Wholly Compensatory Nursing System based on NANDA taxonomy to manage total self-care deficit.
Case Report: A 58-year-old Thai male was found unconscious after a motorcycle accident. The ALS team provided continuous care during transit. Upon ER arrival, the patient was comatose with a GCS of 4 (E1V2M1), exhibiting stridor and decreased breath sounds.
Conclusion: Five primary nursing problems were identified: 1) life-threatening risk, 2) impaired gas exchange, 3) risk for increased intracranial pressure, 4) risk of transfer complications, and 5) family anxiety. Hypoxia was promptly managed through endotracheal intubation. The patient received continuous nursing care until vital signs stabilized and was subsequently transferred safely to Phutthasothon Hospital for definitive and advanced care.
Implications: A Clinical Nursing Practice Guideline (CNPG) should be developed for patients with multiple trauma by integrating systematic emergency assessment, holistic care, and Orem’s wholly compensatory nursing system, with an emphasis on the early identification and continuous monitoring of life-threatening conditions associated with chest trauma.
