Development of Clinical Nursing Practice Guideline for Patient with Radial Artery Catheterization Procedure

Abstract

This research and development aimed to develop the clinical nursing practice guideline for patients with radial artery catheterization procedures and evaluate the outcomes of using the guideline. The samples were purposively selected. They were the guideline developers; 5 nursing administrators and 5 nursing specialists, the guideline users; 20 professional nurses worked at the cardiac intensive care unit and 113 patients with acute myocardial infarction who underwent the radial artery catheterization. The patients were divided into two groups, control group (56 patients received the usual care) and experimental group (57 patients received the developed nursing practice guideline). The research was conducted into 6 steps; situation study of nursing care for patients received cardiac catheterization procedure, development of nursing practice guideline, guideline trial, guideline improvement, guideline Implementation and outcome evaluation. Data were analyzed using the descriptive statistics and Chi–square test.

The results found that 1) the developed nursing practice guideline comprised of 2 phases, phase 1, before radial artery catheterization; patients assessment according to the focus system, information providing before radial artery catheterization, risk assessment for complications, patients preparation before the procedure and nursing care during transfer patients to the cardiac catheterization room, and phase 2, after radial artery catheterization; patients assessment according to the focus system, nursing care while patients on, off and after off TR– Band and predischarge education and 2) the outcomes after nursing practice guideline implementation, the experiment group had the significantly lower incidence of limb ischemia than that of control group (p < 0.05). However, there was no statistically significant difference in the bleeding and hematoma among two groups (p > 0.05). The nurses’ satisfaction was at the highest level (mean 4.59, S.D. 0.51).
