Development of a Care Model for Severe Pneumonia in Adults by the FASTHUG and SAR Assessment in the Respiratory Intensive Care Unit, Sakon Nakhon Hospital

Abstract

The action research aimed to develop the care model for severe pneumonia in adult patients and to assess the outcomes of using the care model by FASTHUG and SAR measurement in the respiratory intensive care unit of Sakon Nakhon hospital. The sample groups were 14 registered nurses and 30 adult patients with severe pneumonia. The research was conducted between August to October, 2022. The research instruments consisted of the data collection tools and the research tool validated by 3 experts. The reliability of FASTHUG and SAR and the registered nurses’ satisfaction assessment form tested using the Cronbach’s alpha confidences were 0.74 and 0.70, respectively. The quantitative data were analyzed by the frequency, percentage, mean score, and standard deviation. The qualitative data were analyzed by the conducting thematic analysis.

               The results showed that the care model for adults with severe pneumonia using the FASTHUG and SAR assessment form consisted of feeding (F), pain management (analgesia, A), nervous system control (sedation, S), prevention of venous thromboembolism (thromboembolic prevention, T), head of the bed elevation (H), stress ulcer prophylaxis (U), blood glucose control (G), weaning off the ventilator (spontaneous weaning trail, S), prevention of alveoli and lung rehabilitation (aggressive alveolar maintenance, A) and rational use of antibiotics (R). The average satisfaction of nurses after using the care model was at the highest level ( = 4.57, S.D. 4.32).
