การประเมินความตรงและความน่าเชื่อถือของ Clinical Frailty Scale ในการพยากรณ์ ผลลัพธ์หลังผ่าตัดกระดูกสะโพกหัก โรงพยาบาลสกลนคร: การศึกษาแบบไปข้างหน้า

การศึกษาแบบไปข้างหน้านี้มีวัตถุประสงค์เพื่อประเมินความตรงเชิงพยากรณ์และความน่าเชื่อถือระหว่างผู้ประเมินของ Clinical Frailty Scale (CFS) ในการทำนายภาวะแทรกซ้อนหลังผ่าตัดและการเสียชีวิตภายใน 30 วัน ในผู้ป่วยสูงอายุ 60 ปีขึ้นไปที่เข้ารับการผ่าตัดกระดูกสะโพกหักในโรงพยาบาลสกลนคร จำนวน 104 ราย ผู้ป่วยได้รับการประเมิน CFS ก่อนผ่าตัดโดยแพทย์และพยาบาล วิเคราะห์ด้วย ROC curve, binary logistic regression, Spearman's correlation และ Intraclass Correlation Coefficient (ICC)
ผลการศึกษาพบภาวะแทรกซ้อนหลังผ่าตัดร้อยละ 66.35 ค่า overall AuROC ของ CFS เท่ากับ 0.77 (95% CI: 0.67–0.86) สำหรับภาวะแทรกซ้อน และ 0.76 (95% CI: 0.53–0.98) สำหรับการเสียชีวิตภายใน 30 วัน จุดตัด CFS ≥ 5 ให้ความสมดุลระหว่าง sensitivity และ specificity ดีที่สุด ผู้ป่วยที่มีภาวะเปราะบางมีโอกาสเกิดภาวะแทรกซ้อนสูงกว่ากลุ่มไม่เปราะบาง 5.74 เท่า (OR = 5.74, p < 0.001) ความน่าเชื่อถือระหว่างผู้ประเมินอยู่ในระดับดีมาก (ICC = 0.94) CFS จึงเป็นเครื่องมือที่เหมาะสมสำหรับประเมินความเปราะบางก่อนผ่าตัดกระดูกสะโพกหักในบริบทโรงพยาบาลทั่วไปของไทย
