The Effects of Health Literacy Promotion on Reducing Blood Lipid Levels Among Working-Age Adults in Workplaces, Nakhon Si Thammarat Province

Abstract
Background: Dyslipidemia is a major risk factor for cardiovascular disease among working-age adults. This quasi-experimental study aimed to examine the effects of a health literacy promotion program on health literacy, health behaviors, and blood lipid levels among working-age adults in workplaces in Nakhon Si Thammarat Province.
Methods: Sixty participants with total cholesterol levels > 200 mg/dL and/or LDL levels > 100 mg/dL were allocated to an experimental group or a control group based on worksite and time period (non-random allocation), with 30 participants in each group. The experimental group received a 12-week health literacy promotion program, while the control group received usual care. Data were collected using a health literacy and health behavior questionnaire (KR-20 = 0.79 for health knowledge; Cronbach's alpha = 0.84–0.86 for other domains) and blood lipid measurements. Data were primarily analyzed using two-way repeated measures ANOVA.
Results: After the intervention, the experimental group showed significantly improved health literacy and health behaviors (p < 0.001). Two-way repeated measures ANOVA revealed significant group × time interactions for total cholesterol (between-group difference −28.43 mg/dL, p < 0.001) and LDL (between-group difference −17.33 mg/dL, p < 0.001), with the experimental group showing greater reductions than the control group. Triglyceride did not differ significantly between groups (p = 0.552). For HDL, the control group showed significantly greater improvement than the experimental group (between-group difference −4.30 mg/dL, p = 0.016), a finding contrary to the study hypothesis.
Conclusion: These findings suggest that the program improved health literacy and health behaviors, and reduced total cholesterol and LDL levels; however, HDL improved less than in the control group, contrary to the hypothesized direction, warranting cautious interpretation and further investigation in larger samples. The program may be considered for pilot implementation in workplaces with similar contexts.
