Capacity Development of Village Health Volunteers in Caring for Patients with Type 2 Diabetes Mellitus: A Systematic Review

Abstract
Background: Type 2 diabetes mellitus (T2DM) is a major public health problem, with an increasing prevalence worldwide and in Thailand. Village health volunteers (VHVs) play an important role in supporting community-based diabetes care in Thailand. However, evidence regarding approaches to developing the capacity of VHVs to care for patients with T2DM has not been systematically synthesized. This systematic review aimed to synthesize the models, components, processes, and outcomes of capacity development for VHVs in caring for patients with T2DM.
Methods: This systematic review was conducted in accordance with the PRISMA 2020 guidelines. Google Scholar, PubMed, and ThaiJO were searched for studies published between 2020 and 2026 in Thai or English, with the final search conducted on June 25, 2026. Studies were selected according to the PICOS framework by two independent reviewers. Study quality was assessed using Heller’s appraisal tool. Data were synthesized using descriptive content analysis. Meta-analysis was not performed because of substantial heterogeneity in study designs and outcome measures. The review protocol was not registered.
Results: A total of 1,403 records were identified. Of these, 21 studies met the eligibility criteria, of which 12 met the predefined methodological quality threshold, with scores ranging from 19 to 21 out of 22. Capacity development approaches included training programs, learning-based activities, and participatory processes. Key strategies comprised knowledge and skills development, attitudes and motivation strengthening, reflective practice, technology-assisted learning, home visits, mentoring, and coaching. These approaches improved VHVs' diabetes-related knowledge, communication skills, confidence, and positive attitudes toward patient care and were associated with improved patient self-care behaviors.
Conclusion: The capacity development approaches identified in this review may be adapted to the Thai context to strengthen VHVs' capacity to care for patients with T2DM. The findings should be interpreted and applied with consideration of the heterogeneity and methodological limitations of the included studies.
